Keratolytic winter erythema is a rare epidermal disease, characterized by recurrent centrifugal palmoplantar peeling and erythema presenting seasonal variation (cold weather). Skin lesions may spread to the dorsum of hands and feet and to the interdigital spaces. Lower legs, knees and thighs may also be involved. Episodes may be preceded by itch and hyperhidrosis. Skin biopsy reveals an epidermal spongiosis with clefting in the stratum corneum, followed by regrowth. Keratolytic winter erythema follows an autosomal dominant mode of transmission.

A Comparison of Two Different Treatment Approaches for Adolescents With Osgood Schlatter: A Randomized Controlled Superiority Trial (The SOGOOD Trial)

The most common growth-related injury is Osgood Schlatter, which affects up to 1 in 5 physically active adolescents. It can cause long-term pain and potential discontinuation of sports and physical activity, with sequela well into adulthood. No effective conservative treatments have been documented, and clinical practice is characterized by a wealth of conflicting advice and modalities. A novel treatment approach has shown promising results in a small single-cohort study. Therefore, this study aims to compare this novel treatment with usual care in 10-16-year-old adolescents with Osgood Schlatter.
This single-center pragmatic, double-blinded, randomized, controlled superiority trial, will have a two-group parallel arm design. Participants will undergo 3 months of treatment, followed by 2 months of self-management with self-reported knee function (KOOS-child 'Sport/rec') at 5 months as the primary endpoint.
This trial comparing a novel treatment with usual care for adolescents with Osgood Schlatter could result in an evidence-based treatment ready for implementation in clinical practice, benefitting patients outcomes and clinicians.

Use of a poll‐mounted accelerometer for quantification and characterisation of equine trigeminal‐mediated headshaking

AbstractBackgroundEquine trigeminal‐mediated (TGM) headshaking (HS) is a neuropathic facial pain syndrome characterised by varying intensity and frequencies of head movements and signs of nasal irritation. An accurate method for quantification and/or characterisation of HS severity is lacking.ObjectivesTo develop and validate an objective measure of TGMHS.Study designProspective case control study.MethodsHorses presenting for investigation of HS were recruited alongside those presenting for forelimb lameness (LAME) and pre‐purchase examination as well as healthy controls (CONTROL). Head movement data were collected for 5 min whilst trotting on the lunge using a tri‐axial accelerometer, with a range of ±16 g and sampling rate of 800 Hz, attached to the bridle headpiece. Recordings were exported for processing. Peak detection was performed using minimum and maximum thresholds of −1 g and +1 g (corrected for gravity) and a minimum peak width of 10 samples.ResultsFifty‐six horses were included in the study; 18 TGMHS, 10 non‐TGMHS, 12 LAME and 16 CONTROL. Characteristics and frequency of vertical (Z axis) head movements of TGMHS horses differed significantly from other horses. TGMHS horses had peaks with greater mean and maximum positive g‐force (P < 0.005) and lower mean and minimum negative g‐force (P < 0.001), greater frequency of peaks/min (P < 0.001) and over 12 times greater percentage of peaks >+2 g compared with other horses (P < 0.001). Receiver operator curve characteristics of percentage of peaks >+2 g (CI 0.72–0.95), percentage of peaks <−2 g (CI 0.66–0.92) and percentage of peaks <−2 g and >+2 g (CI 0.72–0.96) showed excellent discrimination of TGMHS horses from LAME, CONTROL and non‐TGMHS horses.Main limitationsReferral population of horses, small sample size and control horses were not evaluated for orthopaedic disease.ConclusionsAccelerometer data from trotting exercise on the lunge provides an objective measure of HS and can differentiate between TGMHS, non‐TGMHS, normal head movements and those associated with forelimb lameness. Accelerometer use may aid HS diagnosis and monitoring of management strategies.

Outcomes of triple-class refractory multiple myeloma patients at a single clinic: A retrospective study

BACKGROUNDMultiple myeloma (MM) is an incurable B cell malignancy requiring complex treatments with a variety of drugs. Patients often become refractory to proteasome inhibitors, immunomodulatory agents, and anti-CD38 monoclonal antibodies (mAb), and termed triple class refractory (TCR), associated with a poor prognosis.OBJECTIVEWe performed a retrospective analysis of data regarding the outcomes of TCR MM patients at a single clinic specializing in the care of multiple myeloma patients.PATIENTS AND METHODSA retrospective chart review was conducted among 117 patients with TCR MM at a single clinic. Patients' progression-free survival (PFS), overall survival (OS), cytogenetics, lines of prior therapy (LOT), and first treatment they received after the development of TCR disease were determined.RESULTSThe median time from diagnosis to development of TCR disease from diagnosis was 47 months (range: 1.5-191.0; n = 104) and the median LOT prior to TCR disease development was 5 (range: 2-13; n = 117). From the development of TCR disease, patients in our study showed a median OS of 18.9 months (range: 0.03-60.0) and a median PFS of 3.1 months (range: 0.1-55.1). The most commonly utilized first treatment regimens used after TCR disease development were mAb-containing combinations (n = 42). Venetoclax-based regimens showed the longest median PFS at 12.4 months (n = 14).CONCLUSIONSThis median OS of 18.9 months recorded at a single clinic provides real-world data regarding the survival of TCR MM patients highlighting the need for improvement in prognosis for this subset of patients.

Targeting the lncRNA GAS5/TLR4/NLRP3 signaling cascade inhibits endometrial stromal cell pyroptosis and prevents the progression of intrauterine adhesions

Intrauterine adhesion (IUA) poses a serious threat to women's health, and its specific pathogenesis has not yet been elucidated. Our study found through high-throughput sequencing that differentially expressed genes of the endometrial tissues from healthy individuals or IUA patients were enriched in the toll-like receptor (TLR), nuclear factor-kappa B (NF-kB), and nucleotide-binding oligomerization domain-like receptor (NLR) signaling pathways. Meanwhile, we observed that compared to the controls, long non-coding RNA (lncRNA) growth arrest-specific transcripts 5 (GAS5) was significantly upregulated in the endometrial tissue of IUA patients and scratching/lipopolysaccharide (LPS)-induced IUA model mice. Subsequently, results from the functional verification assay, including hematoxylin-eosin staining, enzyme-linked immunosorbent assay, and western blot, showed that knockdown of GAS5 improved endometrial injury and uterine adhesions, decreased the levels of TIMP1, α-SMA, Vimentin, and COL1A1, but elevated MMP9 level to reduce excessive accumulation of extracellular matrix (ECM), and inhibited the expression of NLRP3, cleaved caspase-1, GSDMD, and nuclear p65 to ameliorate pyroptosis in IUA model mice. As confirmed by bioinformatics analysis and dual luciferase reporter gene system, GAS5 sponged microRNA (miR)-205-5p to upregulate TLR4, further activating the NF-kB and NLRP3 signaling in endometrial stromal cells (ESCs). The in vitro functional recovery experiments suggested that GAS5 knockdown alleviated LPS-induced activation of the NF-kB and NLRP3 signaling, pyroptotic cell death, and ECM deposition in ESCs, which was counteracted by overexpressing TLR4 and NLRP3. In a word, our study proved that targeting the GAS5/TLR4/NLRP3 signaling cascade inhibits ESCs pyroptosis and prevents the progression of IUA, providing promising therapeutic strategies for IUA disease.
