OutcomesAI raised $10 million in seed funding to cut down on nursing costs,

The Boston-based startup is made of two parts: AI voice agents that handle routine patient interactions like phone calls, scheduling visits and follow-ups (though it refers more serious cases to human nurses), and AI-powered administrative tools to support nurses, from reviewing charts to scribing interactions and guidance with following protocols. Santé Ventures led the seed round.
The goal is to help health systems and virtual care companies save money by freeing up nurses to handle more tasks with the tools, reducing the number of nurses companies need to employ, CEO and founder Kuldeep Singh Rajput told Endpoints. Rajput, who previously founded remote patient monitoring startup Biofourmis and served as CEO until he stepped down in 2023, said he was inspired by the problems he saw working there.
“When we take a closer look at what these nurses were doing back at Biofourmis — charging the patients, triaging symptoms, checking vital signs, coordinating follow ups, documenting notes, and so on — almost just 30% was spent on direct patient care,” he said. “This insight really stayed with me, and in fact, that was one of the reasons I started OutcomesAI.”
Biofourmis, which started in 2015, hit a valuation of $1.3 billion in 2022 after a $300 million Series D backed by General Atlantic. Rajput stepped down about a year later following
layoffs
at the company, which he said was a sign of the company’s growth and his time to leave.

“I enjoy building and innovating, and I think it was the right time for me to step down and focus on building the next company,” he said.
Rajput said he hopes to sell OutcomesAI to health systems and virtual care companies. Health systems tend to be short-staffed and spend heavily to bring in outside help, while virtual care companies are under pressure to expand without increasing their costs, he said. OutcomesAI is currently working with five clients.
While other companies have developed voice AI agents or tools for specific administrative tasks, Rajput argues that OutcomesAI stands out by offering an all-in-one solution. Its customers also have the option to work with nurses employed by OutcomesAI who are already trained on safety protocols and how to use its tools. This lets them skip the messy work of having to train their own staff on a new platform. Like Biofourmis, OutcomesAI also hires staff and has offices in Singapore and India as good quality labor can be cheaper overseas.
Due to the ongoing nursing shortage and the slow growth of many virtual care programs, there’s growing demand for a model that integrates AI with human nurses, Rajput said.
So far, OutcomesAI is focusing on triage and virtual care programs, like remote patient monitoring and chronic care. It’s currently developing a nursing line for specialty care programs like cardiology and dementia, and pharmaceutical companies.
