CDS微专辑 扫描二维码 可查看更多内容 编者按 随着社会的发展和生活方式的改变，糖尿病及其并发症已成为全球性的公共卫生问题。在中华医学会糖尿病学分会第二十六次全国学术会议（CDS2024）期间，本刊对天津医科大学朱宪彝纪念医院陈莉明教授进行专访。陈教授基于临床经验和团队的科研成果，为我们揭示了糖尿病肾脏病（DKD）的发病机制、治疗策略及未来研究方向。随着对糖尿病认识的不断深入和研究持续进展，我们有理由相信，这将为广大糖尿病患者带来更多获益。 《国际糖尿病》 糖尿病在我国有着庞大的患者人群，这类患者往往同时伴有多种并发症，比如糖尿病肾病就是非常庞大的一类合并症，请您介绍一下DKD人群现在的诊疗情况？这类患者数量有多少？关于治疗方案有哪些？ 陈莉明教授：近年来，我国糖尿病患者率持续攀升，根据国际糖尿病联盟（IDF）的第10版数据，截至2021年，我国20~79岁人群的糖尿病患病率为10.6%，患病人数达到1.41亿，糖尿病的主要负担来自于糖尿病的慢性并发症，而糖尿病肾脏病是糖尿病重要的微血管并发症。 从贾伟平院士在本次CDS会议上所作的“糖尿病防治体系的建立及实践”报告中，我们了解到目前中国糖尿病伴发慢性肾脏病（CKD）人群的严峻形势：患病率高达32.7%，性别间差异不显著，但呈现出明显的地域性特征，即南方（35.2%）高于北方（30.2%）；农村地区的患病率（34.8%）亦高于城市（30%），这一趋势或许与地域性的饮食结构和生活方式紧密相关。因此，对于DKD患者的管理，我们强调早期诊断的重要性，以期预防和延缓疾病的进展。 DKD在早期通常没有明显的症状，容易被患者忽略，常在被发现前就已进展至中末期，造成不可逆转的后果。为了做到DKD的早发现、早治疗，《国家基层糖尿病肾脏病防治技术指南（2023）》建议基层医疗卫生机构应在2型糖尿病患者确诊时进行CKD筛查，并每年至少进行一次筛查。 对于DKD患者，一般治疗方案包括生活方式干预、控制血糖、管理血压和血脂以及使用具有DKD获益类的药物（如nsMRA）。在控制血糖方面，推荐患者使用降血糖效果良好，同时具有额外的肾脏保护作用的药物如有循证证据的SGLT2抑制剂（SGLT-2i）和GLP-1受体激动剂。 《国际糖尿病》 SGLT-2i是当前治疗DKD疾病中常用到的一类药物，请问这类药物的获益有哪些？这类药物在肾脏保护方面您认为有没有类效应？ 陈莉明教授：SGLT-2i是一种通过抑制肾脏近端小管中的SGLT-2作用，减少葡萄糖的重吸收，增加尿糖排泄，从而实现降低血糖的一类药物。由于其独特的胰岛素非依赖性机制，这类药物有效降低了低血糖的发生风险，同时伴随减重和降压效果。 随着研究的不断深入，越来越多证据表明这类药物在降糖同时，具备心血管获益，比如降低重大心血管事件（MACE）、对于心衰患者获益证据更充分。同时，还显示具有肾脏获益，延缓肾脏病的进展。具体到肾脏保护的机制包括降低血糖、降低血压、恢复管-球反馈、减轻免疫炎症和氧化应激反应、改善细胞自噬、纠正缺氧、改善肾脏能量代谢及血流动力学等。我们知道这类药品目前上市的已经有6种，其中已知恩格列净的EMPA-REG OUTCOME研究、达格列净的DAPA-CKD研究、卡格列净的CANVAS 研究，。此外，脯氨酸恒格列净作为“十二五”国家科技重大专项中的化学创新药，有效降低HbA1c 0.94%，在透析患者人群进行的I期临床研究已证明其安全性，同时针对CKD人群的HERO-CKD研究正在进行中，我们期待它更多临床获益的结果。 《国际糖尿病》 对于DKD，我们了解到您的科研团队也做了相关的机制研究，可以为我们简单介绍一下吗？这些研究对未来治疗DKD患者有哪些启示？ 陈莉明教授：我们课题组一直致力于糖尿病并发CKD的研究，深入探讨了高糖、高脂等多种相关因素。近期研究的焦点集中在支链氨基酸（BCAAs）与DKD进展之间的紧密联系。前期研究发现，早期糖尿病肾脏病患者的血液中BCAAs水平显著升高，因此，BCAAs稳态破坏可能是DKD发生的风险因素之一。 db/db小鼠是研究2型糖尿病肾损伤的成熟模型。本课题组研究发现，与对照组小鼠相比，db/db小鼠表现出肾小球基底膜增厚、基质堆积、炎症细胞浸润及肾功能受损等特征。通过肾脏组织RNA测序分析发现，缬氨酸、亮氨酸和异亮氨酸的降解途径在db/db小鼠中显著富集。同时，BCAA分解代谢相关基因整体下调。结果表明，升高的支链氨基酸可能是引发DKD的重要致病因素。 BCAA分解代谢缺陷促进了DKD的发展，加剧肾脏炎症和EMT（上皮-间质转化）过程。使用BCKDK抑制剂后，通过降低BCKDHA磷酸化来增强BCAA分解代谢，显著降低BCAA水平。同时，减轻肾脏炎症、纤维化等病理状态，从而有效缓解肾脏疾病的进展。 这一研究结果为DKD患者的治疗策略提供了新思路，包括药理学干预和饮食管理等方面：一方面，通过开发针对BCAA代谢途径的调节剂，尝试阻断或逆转DKD进程；另一方面，倡导个性化营养干预，低BCAA饮食与适量运动相结合，以减轻肾脏负担，延缓疾病进展。 当然，从基础研究到临床应用的转化是一个漫长而复杂的过程。但我们坚信，通过科研人员、临床医生以及患者自身的共同努力，以及社会各界的支持与理解，我们一定能够推动这一领域的研究不断向前发展，为DKD患者带来更多的治疗希望。 声明：本文仅供医疗卫生专业人士了解最新医药资讯参考使用，不代表本平台观点。该等信息不能以任何方式取代专业的医疗指导，也不应被视为诊疗建议，如果该信息被用于资讯以外的目的，本站及作者不承担相关责任。 最新《国际糖尿病》读者专属微信交流群建好了，快快加入吧！扫描左边《国际糖尿病》小助手二维码（微信号：guojitnb），回复“国际糖尿病读者”，ta会尽快拉您入群滴！ （来源：《国际糖尿病》编辑部） 版权声明 版权属《国际糖尿病》所有。欢迎个人转发分享。其他任何媒体、网站未经授权，禁止转载。

引言 免疫系统需要控制免疫反应以在抑制感染和预防过量炎症对组织的损伤之间保持平衡【1】。作为循环系统中数量最多的免疫细胞类型，中性粒细胞对于一大部分的病原体具有重要的防御作用，同时为了维持免疫反应平衡，中性粒细胞的抗微生物反应必须被准确调控【2,3】。中性粒细胞被促炎信号激活，并依据表面受体组成和内源蛋白的不同开启不同的效应作用。比如，中性粒细胞吞噬微生物后会产生活性氧、发生脱颗粒或者释放NETs（Neutrophil extracellular traps，中性粒细胞胞外杀菌网络）【4】。NETs是一种细胞外结构，主要由DNA、颗粒蛋白、核蛋白等组成，这些物质组装在去凝集的染色质支架上。NETs能够包裹、中和并杀死微生物，包括真菌、细菌和寄生虫等。不过其它一些刺激信号如晶体和免疫复合物等也能诱导NETs的形成。值得注意的是，在一些自身免疫疾病中，中性粒细胞会被活化并释放NETs，如类风湿性关节炎（rheumatoid arthritis, RA）和系统性红斑狼疮（systemic lupus erythematosus, SLE）等【5,6】。NETs含有许多分子能够被免疫细胞的受体识别。例如，巨噬细胞和树突细胞吞噬NETs后，其胞内的cGAS（cyclic GMP-AMP synthase）能够识别NETs中的DNA骨架。另外，细胞表面受体TLR9也能够识别NET-DNA。一些研究也发现，NETs能够激活一些免疫细胞的膜受体，但是其作用机制还未研究清楚。 近日，来自英国埃克塞特大学的Gordon D. Brown团队在Nature发表了文章Recognition and control of neutrophil extracellular trap formation by MICL，他们发现受体MICL（myeloid inhibitory C-type lectin-like）缺失能够通过促进中性粒细胞NETs的形成进而促进类风湿性关节炎的进展。 研究人员利用Micl-/-小鼠构建了类风湿性关节炎的CAIA（collagen antibody-induced arthritis）模型，Micl敲除小鼠表现出了增强以及非缓解的关节炎表型。研究人员取野生小鼠疾病进展最明显的、造模第7天的小鼠关节组织进行分析。通过流式细胞检测发现，关节组织中浸润的中性粒细胞在Micl-/-小鼠中特异性增多。关节组织中其他细胞类型的数量没有发生改变，另外，骨髓以及血液中中性粒细胞的数量也没有发生改变。这一结果在RA的另一个K/BxN模型中也得到了类似的结果。 为了探究中性粒细胞在RA进展中的作用以及和MICL的关系，研究人员在CAIA模型中应用了anti-Ly6G以清除循环中性粒细胞，发现野生小鼠和Micl-/-小鼠的疾病进展下降到了同一层次，说明Micl敲除引起的RA进展是通过中性粒细胞来实现的。进一步的，研究人员分别从WT以及Micl-/-小鼠的骨髓中分离了中性粒细胞，然后在CAIA模型的第7天将这些中性粒细胞分别移植到RA模型小鼠中，研究人员观察到来自Micl-/-小鼠的中性粒细胞加重了关节炎症。这一结果更加证明了Micl-/-小鼠中中性粒细胞在RA进展中发挥了关键作用。 接下来，研究人员发现Micl-/-小鼠来源的中性粒细胞在尿酸钠（monosodium urate, MSU）或者A.fumigatus hyphae刺激下ROS水平提高，由于NET的形成与ROS水平相关，研究人员检测发现Micl缺失导致NET形成也相应增强。NADPH氧化酶抑制剂DPI（diphenyleneiodonium chloride）抑制了NET的形成，说明Micl缺失引起的NET形成依赖于NADPH氧化酶的活性。除了ROS之外，NET的形成也需要组蛋白的瓜氨酸化修饰，这种修饰依赖于PAD4蛋白的功能。而PAD4抑制剂能够在体外实验中抑制NET的形成，体内实验也显示PAD4抑制剂导致Micl-/-小鼠关节中NET的数量明显减少，但并没有影响关节中中性粒细胞的浸润。 研究人员之前的研究已经发现MICL的单克隆抗体能够加剧野生小鼠CAIA模型的病理程度，类似于Micl-/-小鼠的表型。体外实验也显示，MICL的单抗在中性粒细胞中引起了MSU介导的ROS产生增加。另外，MICL单抗对CAIA模型小鼠病理的刺激作用能够被PAD4抑制剂所抑制。这在RA的其他动物模型如CIA（collagen-induced arthritis）中得到了验证。研究人员进一步检测发现，人体MICL抗体能够促进中性粒细胞的活化以及NET的形成。来自RA病人的血清同样能够促进ROS的产生，说明RA病人血清中含有的MICL抗体抑制了MICL的功能，促进了中性粒细胞的活化以及NET的形成。 于是，研究人员进一步检测了RA病人血清中MICL抗体水平和RA病理程度的关系，发现RA病人中MICL抗体的整体水平比对照组高。另外，在与中性粒细胞NET相关的疾病如系统性红斑狼疮和COVID-19的病人血清中，MICL的抗体水平也同样比正常人高，说明MICL被抑制后，中性粒细胞的活化与这些免疫疾病的进展密切相关。 最后，研究人员发现MICL是一个PRR受体，能够识别NET中的基因组DNA，且这种识别作用能够被MICL的抗体打断。 模式图（Credit: Nature） 综上，本研究发现受体MICL能够调控自免疫疾病中中性粒细胞的活化，一些自免疫疾病病人体内产生过多的MICL抗体，抑制了MICL的功能，导致中性粒细胞持续活化，打破了机体的免疫平衡。本研究确认了自身免疫疾病中免疫平衡维持的关键分子机制，为相关疾病的治疗提供了新的思路和靶点。 参考文献 1. First in-vivo gene replacement therapy program for MSUD MSUD is a severe genetic disease with liver transplantation as the only treatment currently available MILPITAS, Calif.--(BUSINESS WIRE)-- ASC Therapeutics, a privately-held biopharmaceutical company pioneering the development of transformative in-vivo gene replacement, gene editing and allogeneic cell therapies, today announced a partnership with the University of Massachusetts Medical School (UMMS) to co-develop a gene therapy for Maple Syrup Urine Disease (MSUD). MSUD is caused by biallelic mutations in one of three genes that encode subunits of the branched-chain ketoacid dehydrogenase complex (BCKDC), namely BCKDHA, BCKDHB, and DBT.
