Assessment of Ankle Mobility, Flexibility, and Y-Balance Test Before and After Plantar Fascia Stretching and Plantar Fascia Stretching Combined With Graston Technique in Recreationally Active Young Adults: A Randomized Controlled Study

This study investigates the effects of plantar fascia stretching and plantar fascia stretching with graston application on ankle mobility, flexibility and Y-Balance test results in recreationally active young adults. By comparing the effects of the two treatment methods on ankle function, it aims to demonstrate the potential benefits of both treatments.
It was intended to involve 60 participants, or 95% of the interval 0.80's study power.

EFFECT OF SELF-MYOFASCİAL RELEASE ON MUSCLE MECHANİCAL PROPERTİES, PERFORMANCE, NEUROMUSCULAR FUNCTİON AND MİNİMİZATİON OF İNJURY RİSK İN YOUNG SOCCER PLAYERS

The aim of the study is to evaluate the effect of myofascial self-release on muscle mechanical properties, performance, neuromuscular function, and injury risk reduction in young soccer players. The hypothesis is a reduction in the number of injuries in the experimental group, as well as improvements in muscle mechanical properties, performance, and neuromuscular function in young soccer players.The study will be a randomized clinical trial with a convenience sample of approximately 65 male athletes aged between 14 and 18 years, divided into control and experimental groups. Athletes with no history of injury in the three months prior to the study will be included, while those with a history of previous surgery or lower limb fractures in the past five years, or who are undergoing physiotherapy at the time of the study, will be excluded.Muscle mechanical properties (myotonometry), flexibility (sit-and-reach test), ankle dorsiflexion range of motion (Weight Bearing Lunge Test), motor control (Y Balance Test), and the reactive strength index (Myjump App) will be assessed before, immediately after, and at 8 and 16 weeks post-intervention. Myofascial self-release will be performed bilaterally on the quadriceps, hamstrings, and calves for 2 minutes per region, twice a week, using a structured massage roller. All procedures will be demonstrated beforehand during the study.
For the Myoton procedure, the athlete will simply lie in a supine and prone position on the examination table. The athlete will only be asked to remain still in the "face-up" or "face-down" position, feeling only the light contact of the device's probe, which is painless and will be demonstrated beforehand.
In the Y Balance Test, athletes will be asked to balance on one foot atop a grid placed on the floor, with their hands on their hips, and reach as far as possible with the non-supporting leg in three marked directions on the floor, without lifting the heel of the supporting leg.
For the Ankle Range of Motion - Weight Bearing Lunge Test, the athlete will place the tested foot on a measuring tape positioned on the floor and will be instructed to touch their knee to the wall in front of them without lifting the heel of the tested foot. Three attempts will be allowed, and the longest distance from the big toe to the wall will be recorded.
In the Sit-and-Reach Test, the athlete will sit on the floor with bare feet and both legs extended. They will be instructed to keep their hands side by side and try to reach the greatest possible distance on the ruler in front of them by bending forward without bending their legs, holding the position for one second to measure the distance. Three attempts will be allowed, and the greatest distance reached will be recorded.
For ground reaction strength, the athlete will be asked to jump off a 40 cm high box, and as soon as their feet touch the ground, they will be encouraged to jump upward as quickly and as high as possible.
In myofascial self-release, athletes in the experimental group will be asked to use the myofascial release roller for 2 minutes before training on each of the three muscle groups - anterior and posterior thigh and posterior leg. Athletes will be instructed to roll using as much pressure as possible on the roller. For the anterior thigh, they will roll across the entire front part of the thigh; for the posterior thigh, they will roll from just below the gluteal region to just above the knee; and for the posterior leg, they will roll from just below the knee to just above the heel.

Effects of Post Isometric Relaxation and Post Facilitation Stretch on Pain, Range of Motion and Disability in Patients With Tight Hamstrings Syndrome

This randomized clinical trial aims to compare the effects of post-isometric relaxation (PIR) and post-facilitation stretch (PFS) on pain, range of motion, and disability in individuals with tight hamstring syndrome. Conducted at Riphah Rehabilitation Clinic, Lahore, participants aged 18-35 years with posterior thigh pain will be divided into two groups. Both groups will receive hot pack therapy and core strengthening exercises, with Group A receiving PIR and Group B receiving PFS. Outcomes will be assessed at baseline and after 4 weeks using NPRS, a disability questionnaire, and a goniometer, with data analyzed using SPSS version 25.

Mercury Exposure in Goldsmithing: Unveiling Occupational Hazards

Mercury poisoning in the workplace has long been recognized as an occupational disease for several decades. Elemental (or metallic) mercury can cause intoxication, mainly through inhalation, affecting workers in mining operations and goldsmiths engaged in gold recovery processes. We present the case of a professional goldsmith, a smoker with a history of narcolepsy and catalepsy. He experienced an occupational accident resulting in respiratory and mucocutaneous symptoms. The confirmation of the diagnosis by mercury measurements on the blood/urine prompted the start of chelation treatment, which led to favorable progression with symptom resolution. This case underscores the importance of public awareness to prevent mercury poisoning and emphasizes the need for healthcare providers to promptly recognize such occupational health risks.

Fumaria vaillantii extract protects PC12 cells against neurotoxicity induced by 6-OHDA

BACKGROUNDParkinson's disease is a neurological disorder caused by the loss of dopaminergic neurons in the midbrain. Various mechanisms are involved in the incidence of the disease including oxidative stress. Several herbs and natural products may interfere with the oxidative-stress pathway due to their antioxidant effects.OBJECTIVEHerein, we aimed to investigate the neuroprotective role of F. vaillantii extract on Parkinson's in vitro and in vivo model owing to the presence of the bioactive agents with antioxidant properties.METHODSIn vitro experments showed that 6-hydroxydopamine could induce toxicity in PC12 cells. The impact of F. vaillantii extract on cell viability was measured by using MTT assay. Nuclear morphological changes were qualitatively evaluated employing Hoechst staining. The antioxidant activity of the extract was determined by ROS and lipid peroxidation assays. Tyrosine hydroxylase protein expression was measured by western blotting in PC12 cells. For in vivo study, movement parameters were evaluated.RESULTSThe results indicated that 75 µΜ of 6-OHDA induced 50% toxicity in PC12 cells for 24 h. Following post-treatment with F. vaillantii extract (0.1 mg/ml) for 72 h, we observed that the extract effectively prevented cell toxicity induced by 6-OHDA and reduced the apoptotic cell population. Furthermore, the extract attenuated the ROS level, lipid peroxidation and increased protein expression of TH after 72 h of treatment. In addition, oral administration of 300 mg/kg of F. vaillantii extract for 14 days improved locomotor activity, catalepsy, bradykinesia, motor coordination and reduced the apomorphine-caused rotation in 6-OHDA- induced Parkinson's disease-like symptoms in male rats.CONCLUSIONThe present study suggests a protective role for the extract of F. vaillantii against oxidative stress-induced cell damage in the PC12 cells exposed to neurotoxin 6-OHDA which was verified in in vivo model by reducing the motor defects induced by 6-OHDA. This extract could be a promising therapeutic agent for the prevention of PD progression.

Acute Catatonia Following a Cerebellar Stroke: A Case Report

Catatonia can present with a wide spectrum of psychomotor symptoms and should be considered in the differential diagnosis of hospitalized patients with speech and motor difficulties. Catatonia is defined as the presence of three or more of the following: catalepsy, waxy flexibility, stupor, agitation, mutism, negativism, posturing, mannerisms, stereotypies, grimacing, echolalia, and echopraxia. In this case, a 72-year-old black woman was admitted with difficulty with speech and ambulation and found to have a cerebellar stroke on a brain MRI. However, her symptoms of variable rigidity, mutism, and marked psychomotor slowing were not attributable to the small left-sided cerebellar infarction on imaging. A dramatic response to a lorazepam challenge confirmed a diagnosis of acute catatonia secondary to a medical condition.

点击“蓝字”关注我们编者按：2025年4月11日是第29个“世界帕金森病日”，帕金森病（PD）是一种常见的神经退行性疾病，表现为运动障碍（震颤、僵硬、运动迟缓）以及非运动症状（认知障碍、抑郁等），主要病理改变是黑质多巴胺能神经元的丢失及路易小体的形成。而2型糖尿病（T2DM）是一种慢性代谢疾病，以高血糖和胰岛素抵抗为特征。帕金森病和T2DM都是与年龄相关的慢性疾病，会严重影响老年人的生活质量。尽管这两种疾病表面上分属不同系统，近年来大量研究显示，它们在流行病学上存在共病趋势，并可能存在共同的病理生理机制。T2DM与PD发病风险增加显著相关T2DM和PD均为与衰老相关的慢性疾病，世界人均预期寿命的延长导致这两种疾病在老年人群中日益流行，给患者、家庭和社会带来了巨大的经济和心理负担。PD是仅次于阿尔茨海默病（AD）的最常见神经退行性疾病，也是过去二十年中增长最快的神经系统疾病之一。PD是一种多因素疾病，但其潜在机制及各种因素之间的相互作用仍未完全明确。近年来，T2DM 被认为与神经退行性变相关，既是疾病发作的风险因素，也是疾病进展的潜在调节因素[1]。多项流行病学研究、实验研究和临床观察提示T2DM可能是PD的风险因素，并可能影响PD的发展和进展[2~4]。这一发现不仅为理解PD的病因学提供了新的视角，也为潜在的预防和治疗策略开辟了新的方向。多项大规模流行病学研究一致表明，T2DM与PD发病风险增加显著相关。一项涵盖2990万受试者和86 345例PD病例的系统评价和荟萃分析显示，与非糖尿病患者相比，糖尿病患者PD的相对风险（RR）为1.27（95%CI：1.20~1.35），表明糖尿病患者发生PD的相对风险增加27%[5]，这一关联在不同地理区域、性别及多个亚组分析中均保持一致。一项基于51 552例芬兰男性和女性的前瞻性研究也进一步支持了这一观点，在平均18.0年的随访期间，研究结果发现，与无T2DM相比，合并T2DM的男性其发生PD的年龄和研究年份调整后风险比为1.80（95%CI：1.03~3.15），女性为1.93（95%CI：1.05～3.53）[6]。这些结果在进一步调整体质指数、收缩压、总胆固醇、教育水平、休闲时间体力活动、吸烟、酒精饮用及咖啡和茶消费后仍然保持稳定。研究还发现T2DM与PD风险之间存在剂量-反应关系。一项纳入15 168 021例40岁及以上成年人的全国性队列研究显示[7]，与无糖尿病组相比，空腹血糖受损（IFG）组发生PD的调整风险比为1.038（95%CI：1.009~1.067），糖尿病病程<5年组为1.185（95%CI：1.143~1.229），糖尿病病程≥5年组为1.618（95%CI：1.566~1.672）[7]，这表明随着糖尿病病程的延长，PD风险逐渐增加。T2DM与PD更快进展有关除了增加PD发病风险，研究还发现T2DM可能加速PD的症状进展。一项研究评估了T2DM对PD疾病进展的影响[8]，结果表明，与没有T2DM的PD患者相比，合并T2DM的PD患者可能面临更快的运动速度减慢和认知能力下降的问题。该研究共纳入167例（8.7%）合并T2DM的PD患者 (PD + T2DM)，1763例（91.3%）PD（无T2DM）患者。控制混杂因素后，合并T2DM的PD患者有更严重的运动症状（运动障碍社会统一帕金森病评定量表第三部分） (25.8 vs. 22.5，P= 0.002）和非运动症状（非运动症状评定量表总分）(38.4 vs. 31.8，P< 0.001)，合并T2DM的PD患者更有可能报告丧失独立性（优势比2.08；P= 0.001）和抑郁（优势比1.62；P= 0.015)。随着时间的推移，合并T2DM的PD患者的运动症状进展明显更快（P= 0.012)，出现更糟糕的情绪症状（P= 0.041)，更有可能出现实质性的步态损害（风险比1.55；P= 0.020）和轻度认知障碍（风险比1.7；P= 0.002)。T2DM与PD存在共同病理机制2型糖尿病与帕金森病在多个病理环节存在交集，包括中枢胰岛素抵抗、线粒体功能障碍、慢性炎症反应、蛋白质聚集以及自噬溶酶体系统失调[9]。01中枢胰岛素抵抗胰岛素不仅是一种调节血糖的代谢激素，也在中枢神经系统中发挥关键神经营养和调控作用。正常情况下，胰岛素通过跨越血脑屏障与大脑中的胰岛素受体结合，调节神经元的代谢、可塑性及突触传递。然而，在T2DM中，长期的高血糖及高胰岛素状态，会阻碍胰岛素信号的正常传导，进而引发中枢神经系统的胰岛素抵抗现象。在PD病中，胰岛素信号通路的下调被认为会影响黑质多巴胺能神经元的存活与功能。研究表明，胰岛素可以通过PI3K/Akt路径促进神经元存活，而胰岛素抵抗可能会激活GSK-3β，诱导神经元凋亡。因此，T2DM状态下的中枢胰岛素功能障碍可能直接加剧PD的神经退行性变化。02氧化应激和线粒体功能障碍线粒体是细胞的能量工厂，其功能障碍与多种慢性疾病密切相关。在T2DM状态下，高血糖导致的氧化磷酸化功能失调，使得线粒体过度运作，从而产生大量活性氧（ROS），进而诱发氧化应激反应。而在PD中，线粒体复合体I活性下降被认为是导致黑质神经元凋亡的关键原因。这两种疾病均呈现出线粒体DNA受损、能量代谢受阻以及抗氧化防御机能下降的特征。氧化应激在此过程中起到了桥梁作用，它不仅破坏细胞结构，还促进多巴胺代谢异常和蛋白质聚集。特别是PD中黑质区域由于铁离子丰富和高代谢率，对ROS极为敏感，这可能解释了为何T2DM患者更易出现帕金森病相关的神经退行病变。03慢性炎症与免疫激活炎症是T2DM和PD的重要共通病理过程。在T2DM中，慢性低度炎症的特征体现为肝脏、脂肪组织及胰岛内部促炎细胞因子（诸如TNF-α、IL-6）的浓度上升，进而触发全身性的炎症反应。这些炎症因子可以穿越血脑屏障，激活小胶质细胞，引发中枢神经系统的免疫反应。PD中小胶质细胞激活是病理标志之一，其通过释放炎症因子和ROS直接损伤多巴胺能神经元。T2DM所引起的外周炎症状态可能成为激活脑内免疫反应的“助燃剂”，加速PD的进展。此外，研究揭示NLRP3炎症小体在这两者间可能扮演着桥梁的角色，连接着代谢异常与神经损伤，起着关键的炎症媒介作用。04蛋白质聚集α-突触核蛋白（α-synuclein）的异常聚集是PD的病理学特征之一。研究发现，T2DM可能通过多种机制促进α-突触核蛋白的聚集。一项动物研究显示，长期高血糖可加剧α-突触核蛋白的积累和丝氨酸129（S129）磷酸化。此外，T2DM相关的代谢产物，如甲基乙二醇（MGO），可能直接与α-突触核蛋白相互作用，加速其有害效应。05自噬与溶酶体功能障碍自噬-溶酶体通路是细胞内清除损伤蛋白和受损细胞器的关键机制，其功能障碍在T2DM与PD中均有报道。在T2DM中，高葡萄糖状态下自噬活性下降，导致胰岛β细胞功能恶化。而在PD中，自噬障碍会加速α-synuclein的积聚，加剧神经元凋亡。有研究发现，T2DM患者大脑中LAMP2A（一种溶酶体膜蛋白）表达下降，这一变化可能与PD相似。失去清除异常蛋白的能力，使细胞面临更大毒性压力，也为两种疾病提供了病理交集。与此同时，自噬障碍也与线粒体损伤、炎症反应协同作用，形成恶性循环。糖尿病与帕金森病治疗的交叉进展随着T2DM与PD在病理机制上的联系逐渐明确，一些抗糖尿病药物被发现可能对帕金森病具有治疗潜力[9]，尤其以GLP-1受体激动剂（GLP-1RAs）最受关注。GLP-1RAs不仅能控制血糖，还可穿越血脑屏障，激活中枢神经保护通路，改善多巴胺能神经元功能。除GLP-1RAs外，中枢胰岛素治疗（如鼻喷胰岛素）也显示出一定的神经保护效果，可能改善PD患者的认知功能。胰岛素增敏剂在动物模型中可抑制神经炎症，但在人群试验中疗效尚不明确。其他如SGLT2抑制剂和DPP-4抑制剂的神经保护潜力也正处于早期探索阶段。总体来看，代谢药物有望作为PD的辅助干预手段，未来，个体化、多机制的联合治疗策略有望同步缓解代谢异常与神经退行性病变，从而为T2DM合并PD患者开辟潜在的全新治疗途径。然而，目前尚未建立用于临床评估脑胰岛素抵抗的标志物，这将是设计和选择未来在 PD 中测试抗糖尿病药物的临床试验患者的有价值工具。结语越来越多的证据表明，T2DM与PD可能存在共同的发病机制，这也为临床预防和治疗PD提供了新的思路。目前糖尿病治疗药物，正在从“血糖控制”逐步迈向“神经保护”，显示出对帕金森病的潜在治疗价值。未来，基于病理共性的个体化治疗策略有望同时改善代谢状态和神经退行症状，为T2DM合并PD患者带来新的治疗希望。参考文献（上下滑动可查看）1.Neural Regen Res. 2025 Nov 1;20(11):3113-3123. doi: 10.4103/NRR.NRR-D-23-01910.
