A polemic in response to Li et al. (American Heart Journal, 2010, 159, pp. 684-690 e1) is presented.Li et al. reported that low-mol.-weight heparin combined with a reduced dose of unfractionated heparin (UFH) when administered as an adjunct in ST-segment elevation myocardial infarction patients undergoing percutaneous coronary intervention (PCI) with drug-eluting stents seems to be safe and efficacious.Yet, several factors belie this conclusion.The dose adjustment of antithrombotics was left to the discretion of the individual physician, resulting in some patients receiving 24,000 U/d of UFH for more than 2 days.This could affect in-hospital outcomes.
