This is a neutrophil granule-derived antibacterial and monocyte- and fibroblast-specific chemotactic glycoprotein. Binds heparin. The cytotoxic action is limited to many species of Gram-negative bacteria; this specificity may be explained by a strong affinity of the very basic N-terminal half for the negatively charged lipopolysaccharides that are unique to the Gram-negative bacterial outer envelope. It may play a role in mediating recruitment of monocytes in the second wave of inflammation. Has antibacterial activity against the Gram-negative bacterium P.aeruginosa, this activity is inhibited by LPS from P.aeruginosa. Acting alone, it does not have antimicrobial activity against the Gram-negative bacteria A.actinomycetemcomitans ATCC 29532, A.actinomycetemcomitans NCTC 9709, A.actinomycetemcomitans FDC-Y4, H.aphrophilus ATCC 13252, E.corrodens ATCC 23834, C.sputigena ATCC 33123, Capnocytophaga sp ATCC 33124, Capnocytophaga sp ATCC 27872 or E.coli ML-35. Has antibacterial activity against C.sputigena ATCC 33123 when acting synergistically with either elastase or cathepsin G.

Atypical pathogen community-acquired pneumonia: an analysis of clinical characteristics, drug treatment, and prognosis in the related patients

INTRODUCTIONSerious respiratory infections can occur in both in-hospital and out-of-hospital settings. These infections are known as community-acquired pneumonias (CAPs). Streptococcus pneumoniae and other microorganisms commonly cause atypical pneumonia. This study examined the clinical features, medication therapy, and prognosis of 85 cases of community-acquired pneumonia (CAP) caused by Mycoplasma pneumoniae (MPP) and Chlamydia psittaci(C. psittaci)neumoniae (CPP).METHODSA retrospective analysis was conducted at Shaoxing People's Hospital from July 2021 to August 2024, using targeted next-generation sequencing (tNGS) of bronchoalveolar lavage fluid (BALF). Patients were classified into the MPP group (54 patients) and the CPP group (31 patients). Compared with the control group, the CPP group had a significantly lower proportion of patients with a contact history of poultry and birds, a shorter length of hospital stay, and a lower percentage of severe pneumonia cases.RESULTSThe MPP group demonstrated higher incidences of cough and sputum production; conversely, the occurrences of fever, fatigue, diminished appetite, and generalised myalgia were comparatively lower. The MPP group exhibited markedly diminished levels of neutrophils, C-reactive protein, procalcitonin, erythrocyte sedimentation rate, heparin-binding protein, alanine aminotransferase, aspartate aminotransferase, lactate dehydrogenase, direct bilirubin, pH, lactic acid, and D-dimer compared with the CPP group. In contrast, the MPP group had a markedly higher lymphocyte count, platelet count, albumin levels, as well as higher concentrations of blood sodium and blood chloride. The drug treatment regimens differed between the two groups, resulting in one unfavourable outcome within the MPP group.CONCLUSIONIn summary, fatigue, fever, and reduced appetite are more prominent symptoms in patients with CPP, whereas cough and sputum production are the primary manifestations of MPP. Pleural effusion is more prevalent in patients with CPP, Additionally, these patients also have increased inflammatory responses and decreased immune function.

Heparin-binding protein levels as an emerging key biomarker for accurate diagnosis of bacterial meningitis: A promising yet preliminary evaluation

This systematic review aims to consolidate evidence on the potential of heparin-binding protein (HBP) as an emerging and promising biomarker for diagnosing bacterial meningitis (BM). We conducted a comprehensive search across PubMed, the Cochrane Library, Web of Science, and China National Knowledge Infrastructure (CNKI) databases, with no restrictions on publication date or language. Sixteen studies, encompassing a total of 2,032 participants, were included in the analysis. The results consistently demonstrated that HBP expression levels in both cerebrospinal fluid (CSF) and blood are markedly elevated in cases of BM compared to patients with non-central nervous system (CNS) infections or other types of meningitis, such as viral meningitis. The pooled sensitivity estimate for HBP measured in CSF was 0.94 [95% confidence interval (CI), 0.88-0.97], and the specificity was 0.96 [95%CI, 0.90-0.98]. The diagnostic odds ratio (DOR) was calculated as 327 [95%CI, 96-1,110]. According to Fagan's nomogram, an initial probability of 20% for a positive test result increased to 85% following a positive HBP test, while a negative test reduced the probability to just 2%. The diagnostic performance of CSF HBP surpassed that of procalcitonin in detecting BM. Furthermore, in cases where symptoms of CNS infection are present, HBP levels in blood also demonstrate notable diagnostic accuracy. These findings suggest that HBP holds significant potential as a biomarker for BM. However, further large-scale studies are required to establish optimal HBP thresholds for improved clinical applicability.

The Predictive Value of Heparin-Binding Protein in Total Joint Arthroplasty Prosthesis Infections

Objective: This study aims to explore the predictive value of heparin-binding protein (HBP) in diagnosing prosthesis infections after total joint arthroplasty (TJA), in order to provide a new biomarker for early identification and management of prosthetic joint infections (PJI) post-TJA. Methods: A retrospective analysis of data from 168 patients who underwent TJA revision at Lianyungang First People's Hospital from October 2020 to March 2024 was conducted. The participants were divided into an infection group (38 cases) and a non-infection group (94 cases). Inclusion criteria included all patients undergoing joint prosthesis revision who had preoperative HBP levels measured. Primary assessment parameters included HBP, C-reactive protein (CRP), white blood cell (WBC) count, and erythrocyte sedimentation rate (ESR). Univariate analysis and multivariate regression analysis were used to evaluate the correlation of these factors with PJI, and the performance of HBP in predicting PJI was analyzed using the receiver operating characteristic (ROC) curve. Results: There were significant statistical differences in HBP, CRP, WBC, and ESR between the infection and non-infection groups (p < 0.05). Multivariate regression analysis showed that HBP is an independent predictive factor for the risk of PJI. The area under the ROC curve was 0.856, indicating that HBP has good predictive performance. The optimal cutoff value for HBP was 51.3, with a sensitivity of 69.2% and a specificity of 89.5%. Conclusion: The study found that HBP levels are significantly associated with the occurrence of PJI following TJA, serving as an effective independent predictive factor for PJI risk. HBP has high predictive value and can be considered an important biomarker for predicting PJI post-TJA in clinical settings, aiding in the early identification and management of PJI, thereby improving patient treatment outcomes and quality of life.
