(Microbial infection) Confers susceptibility to feline leukemia virus subgroup C (FeLV-C) infection in vitro. Heme b transporter that mediates heme efflux from the cytoplasm to the extracellular compartment. Heme export depends on the presence of HPX and is required to maintain intracellular free heme balance, protecting cells from heme toxicity. Heme export provides protection from heme or ferrous iron toxicities in liver, brain, sensory neurons and during erythropoiesis, a process in which heme synthesis intensifies. Possibly export coproporphyrin and protoporphyrin IX, which are both intermediate products in the heme biosynthetic pathway. Does not export bilirubin. The molecular mechanism of heme transport, whether electrogenic, electroneutral or coupled to other ions, remains to be elucidated. Heme b transporter that promotes heme efflux from the mitochondrion to the cytoplasm. Essential for erythroid differentiation.

An Open-label Single Center, Single Participant Study of an Experimental Antisense Oligonucleotide Treatment for PCARP (Posterior Column Ataxia With Retinitis Pigmentosa) Disease Due to Mutations in FLVCR1

The goal of this clinical trial is to evaluate a specific antisense oligonucleotide medication in one patient with posterior column ataxia with retinitis pigmentosa. The main question it aims to answer is: what is the safety and tolerability of this medication in a single participant.

Enamel caries lesions, with and without initial cavitation, in relation to anatomical areas of the proximal surface in deciduous molars

OBJECTIVEproximal enamel caries lesions (PEC) are believed to initiate and progress to cavitation below the proximal contact area (PCA), but no evidence exists on the location of initial carious cavitation on the proximal surface with functional PCA. This study aimed to test the association of anatomical areas of the proximal surface with the severity of PEC and the frequency of cavitation in PEC in primary molars DESIGN: laboratory, observational, transversal study. Exfoliated primary molars (n = 33) with functional PCA (biofilm-free PCA surrounded by biofilm) had their proximal surfaces (one/tooth) divided anatomically into up to nine areas: 3 areas based on the occlusal/cervical PCA boundaries (areas I, II, and III; occluso-cervically) and 3 areas based on the bucco/lingual PCA boundaries (A, B, and C), with area IIB representing the PCA and area IIIB as the sub-PCA (below the PCA). PEC (ICDAS scores 1 and 2-3) and cavitation in PEC were quantified in all areas using stereomicroscopy and microCT. PEC volume was quantified in areas IIB and IIIB under microCT RESULTS: PEC severity increased occluso-cervically. PCA and sub-PCA presented different PEC severities (higher in sub-PCA) and similar PCE volumes, but the odds of carious cavitation were much higher (Odds ratio = 197.4; 95 % CI: 8.7/4480.7) in the PCA than in the sub-PCA (no cavitation).CONCLUSIONPCA presented lower PEC severity and similar PEC volume compared to sub-PCA, but PCA concentrated all cavitations in PEC, supporting a new model for the pathogenesis of PEC.

BACKGROUNDPernicious anemia (PA) is believed to be highly prevalent in Western countries but has rarely been reported in China. The study explores whether PA, an autoimmune disease, is an uncommon cause of cobalamin (vitamin B12) deficiency anemia in China.METHODSClinical and hematological data were collected from 90 cobalamin deficiency-caused megaloblastic anemia (MA) patients between July 2014 and December 2021. Through anti-intrinsic factor antibody (IFA) and anti-parietal cell antibody (PCA) testing, PA was distinguished from other causes of cobalamin deficiency leading to MA. Meanwhile, 30 healthy controls (HCs) were included to estimate the positive rates of IFA and PCA.RESULTSOf the 30 HCs, only one tested positive for IFA, and all 30 tested negative for PCA. Among the 90 patients with cobalamin deficiency-caused MA, 76.7% were positive for IFA, and 47.8% were positive for PCA; a total of 76 patients (84.4%) were diagnosed with PA. The mean follow-up time was 41.0 ± 16.3 months. During the follow-up period, no case relapsed among the continuous cobalamin-supply treatment patients, while 24.4% of patients relapsed due to the interruption of maintenance cobalamin-supplement therapy (the median recurrence time was 54.0 ± 17.7 months).CONCLUSIONSThe proportion of PA in cobalamin deficiency-caused MA patients in Hainan province was higher than 80%, which was more common than expected. Therefore, screening for IFA, PCA, endoscopic biopsy, and thyroid-related parameters are recommended for all cobalamin deficiency-caused MA patients. Furthermore, maintenance cobalamin-supplement therapy is important for PA patients.

Anthocyanin modified by chondroitin sulphate and tannic acid improved the quality-indicating properties of gelatin-based intelligent film

A highly pH-responsive gelatin film incorporating purple cabbage anthocyanin (PCA) and chondroitin sulphate (CS)/tannic acid (TA) was developed. Co-pigmentation of PCA via CS/TA improved its photothermal stability and visibility of color change in gelatin film. The morphological and structural properties of CS-PCA and TA-PCA films revealed that a more stable network was formed as new hydrogen bonds were generated by the co-pigmentation. Meanwhile, the co-pigmentation improved film's mechanical and hydrophobic properties, expressed as higher tensile strength (16.65 and 17.97 Mpa) and lower water vapor permeability (1.45 and 1.41) in CS-PCA and TA-PCA films, compared to PCA film. CS-PCA and TA-PCA films showed distinct color transitions for chilled fish fillets during storage. Total color difference (ΔE) of CS-PCA and TA-PCA films correlated well with the deterioration indexes of total volatile base nitrogen (TVB-N). All the results provided a novel pH-sensitive intelligent packaging strategy by co-pigmenting CS/TA with PCA for freshness monitoring.

《自然》：破解50年谜题！大脑吸收蛋黄中胆碱的转运蛋白，终于找到了丨科学大发现

*仅供医学专业人士阅读参考胆碱是重要的人体必需营养物质。细胞膜的合成和神经信号的传递，都需要大量的胆碱。大脑对胆碱的需求量非常大，而且无法有效地在体内从头合成，因此大脑所需的大部分胆碱都是从我们日常吃的食物中吸收。作为卵磷脂的重要组成部分，胆碱是如何被大脑吸收的这一问题，已经困扰了科学家半个多世纪。前不久，由美国哥伦比亚大学Filippo Mancia和Rosemary J. Cater，以及加州大学旧金山分校Thomas Arnold领衔的研究团队，在顶级期刊《自然》上发表了一篇重磅研究论文[1]，解开了这个困扰科学家长达50多年的学术问题。研究人员首次发现，主要协同转运蛋白超家族转运蛋白FLVCR2，是血脑屏障中主要的胆碱转运蛋白，大脑吸收的大部分胆碱都是通过FLVCR2完成的。他们还借助冷冻电镜，探索了FLVCR2转运胆碱的过程。这一转运蛋白的发现，意义不仅仅在于发现了大脑吸收胆碱的机制，它还意味着日后可以借助于这个吸收过程，开发更多可以进入大脑的药物。 ▲ 论文首页截图近20年来，科学家一直在寻找胆碱的潜在转运蛋白，但是迄今为止，可以确定的胆碱转运蛋白只有两个，一个是几乎只在胆碱能神经元中表达的ChT（又称SLC5A7）[2]；另一种是近两年确定的FLVCR1，它在大多数细胞类型中都有表达，但在脑内皮细胞中表达量却不高[3-5]。从名字上就可以看出，FLVCR1和FLVCR2可能是“一家人”，不过二者其实是近亲，同源性仅为55%。与FLVCR1不同，FLVCR2主要在血脑屏障的内皮细胞中表达。虽然有一些研究发现FLVCR2对脑血管生成和大脑正常发育至关重要，但是FLVCR2的真正生理功能仍不为人所知。 FLVCR1的胆碱转运蛋白身份在2023年被揭示之后[3,4]，近亲FLVCR2的功能也就呼之欲出了。Mancia团队很快就基于小鼠模型证实，FLVCR2确实只存在于所有脑血管节段（动脉、毛细血管和静脉）的内皮细胞中。而且FLVCR2在脑内皮细胞质膜的两侧都有表达，其中管腔那一侧的细胞膜表达水平相对较高。他们在成人大脑中也发现了类似的分布。从表达部位上来看，FLVCR2确实处于将胆碱从血液吸收进大脑的理想位置。 ▲ FLVCR2的空间分布接下来就是探索FLVCR2究竟能不能转运胆碱了。 Mancia团队开发了FLVCR2编码基因Flvcr2被条件性敲除的小鼠模型（Flvcr2-cKO），然后观察被放射性标记的胆碱在体内的吸收情况。结果很明显，在脑血管密度、形态和屏障功能完好无损的情况下，基因缺失小鼠大脑吸收胆碱显著减少。基于体外细胞系的研究也证实了上述发现，所有的证据都表明，FLVCR2确实是血脑屏障中的胆碱转运蛋白，负责大脑对胆碱的吸收。此外，FLVCR2在碱性条件下（pH值7.5以上）比在酸性条件下（pH值7.5以下）更容易结合胆碱。 ▲ 血脑屏障中FLVCR2的示意图在研究的最后，Mancia团队借助冷冻电镜，研究了FLVCR2转运胆碱时，二者之间的相互作用位点。简单来说，作为主要协同转运蛋白超家族的一员，FLVCR2转运胆碱的过程与其他转运蛋白转运底物类似。当胆碱要穿过血脑屏障时，FLVCR2会打开内皮细胞血管腔一侧，闭合胞内一侧；随后胆碱进入腔内与特定位点相结合，引起FLVCR2构象变化——胞内一侧打开的同时，胞外一侧关闭，并完成胆碱的运输。关于这一过程，在Mancia团队的论文见刊的3周之后，由德国马克斯·普朗克生物物理研究所Schara Safarian、Di Wu和Gerhard Hummer，以及新加坡国立大学Long N. 前列腺癌是全球关注的重大健康问题，目前是美国男性中最常见的恶性肿瘤。手术以及放射治疗可用于治疗部分患者，但约三分之一的患者会复发，且这些个体中约三分之一将最终死于前列腺癌。早期前列腺癌的治疗方法：对于局部疾病，手术和/或放射治疗是可行的方法。这些方法可以用于治疗早期前列腺癌。然而，使用这些方法进行治疗，约三分之一的患者仍会出现疾病复发，并且其中约三分之一的个体最终会死于前列腺癌。因此，需要更有效的治疗方法来处理早期前列腺癌。去势疗法是通过抑制男性激素（如睾酮）的产生或作用来治疗前列腺癌。这种方法可以减缓肿瘤生长和扩散，并在一定程度上控制前列腺癌的发展。然而，去势抵抗性前列腺癌（CRPC）通常在去势治疗开始后3年内发生，并且转移性CRPC（mCRPC）是该疾病的致命阶段。在过去的15年中，前列腺癌治疗方面有实质性改进，但大多数这些治疗方法涉及非特异性化学药物或进一步抑制雄激素受体信号的药物。目前可用于处理前列腺癌的后续治疗方案包括化学药物、雄激素受体信号抑制剂、放射治疗、手术和新型免疫治疗方法等。表1 前列腺癌传统治疗方式免疫治疗方法——疫苗治疗的兴起抗肿瘤疫苗是设计用于给癌症患者注射的药物，以引起能够识别和消除肿瘤细胞的免疫反应。抗肿瘤疫苗的方法通常基于已灭活或在某些情况下经过修饰以增加免疫系统对这些细胞的识别的肿瘤细胞的递送;另外，肿瘤疫苗策略使用肿瘤成分的递送来激活针对特定抗原靶点的免疫反应。这些方法依赖于使用编码肿瘤相关抗原靶点的全蛋白、癌症特异性碳水化合物、来自靶蛋白的多肽或遗传疫苗(如病毒、细菌、DNA或RNA疫苗)。前列腺是一个非必需器官，这一特征促使研究人员将抗肿瘤疫苗作为前列腺癌的潜在治疗方法，其基本原理是使用疫苗产生“组织排斥”反应，以消除任何残留的微观癌细胞。与针对重要器官癌症的疫苗相比，这些疫苗的目标不一定是癌症特异性的。此外，前列腺癌通常有很长的自然史，大多数患者从诊断时起存活超过15年。这一证据鼓励了疫苗等疗法的发展，这些疗法可能需要一段时间才能开始起作用，也可能只是改变疾病的发展轨迹。疫苗治疗作为前列腺癌的治疗方法一项以前列腺肿瘤生长速率为模型的研究结果表明，疫苗治疗可能减缓前列腺癌的生长，并比化疗等疗法产生更好的长期结果和生存率，化疗可能只能在治疗期间暂时阻止癌症的生长。此外，与大多数没有血清生物标志物的癌症类型不同，前列腺特异性抗原(PSA)作为前列腺癌的血清生物标志物的可用性使微转移性疾病的检测成为可能，PSA已被用于识别有疾病进展和生存率降低风险的患者。在20世纪70年代，几份报告显示，一些患者在接受冷冻疗法治疗后，前列腺癌转移出现消退，而这促进了前列腺癌疫苗的发展。疫苗可大致分为非抗原特异性疫苗(典型的全细胞疫苗，其特异性抗原靶点未知)和抗原特异性疫苗(图1)。全细胞疫苗方法最初利用了早期微生物疫苗的知识，在这种疫苗中，整个病原体被灭活，然后重新注射到宿主中，试图对病原体呈递的一个或多个抗原产生保护性免疫。这种方法在缺乏已知的肿瘤特异性靶抗原的情况下比较受欢迎，这些抗原在个体之间可能是不同的。非抗原特异性疫苗的一个主要缺点是缺乏确定的靶点作为免疫反应的衡量标准。只有临床反应可以评估，并且没有已知的抗原可用来衡量疫苗接种是否会导致生物学效应。在开发传染性疾病疫苗(如乙型肝炎疫苗)时，使用针对特定抗原的疫苗比使用整个病毒更可取，特别是在病毒无法培养的情况下。这种抗原特异性疫苗产生了高效的保护性免疫，并能够测量个体是否已有效免疫。这些原则有利于抗原特异性疫苗的开发，在大多数情况下，最终目标是开发针对多种确定抗原的多价疫苗。图 1 前列腺癌的抗肿瘤疫苗方法在过去的20年里，通过不同的方法靶向单一或多个靶点抗原的治疗性癌症疫苗作为前列腺癌的治疗方法被广泛研究。这些方法大多仅限于I期研究，几乎所有疫苗都是安全的，并显示出一些免疫活性的证据。在随机III期试验中研究了五种方法——GVAX、DCVAC/PCa(一种多表位肽疫苗)、sipuleucel-T和PROSTVAC(表1)。其中，sipuleucel-T是唯一被FDA批准用于转移性CRPC（mCRPC）患者的免疫治疗方法。这些免疫治疗方法的具体优缺点需要进一步的临床试验来确定。1.GVAX：GVAX是一种基于前列腺癌细胞株的整体细胞疫苗。它的优点是安全性高，且在早期临床试验中显示出一定的抗肿瘤活性。但是，在后期临床试验中并没有显示出明显的治疗效果。2.DCVAC/PCa：DCVAC/PCa是一种个体化的抗原特异性细胞疫苗。它的优点是可以根据患者的个体情况进行定制化治疗，但在大规模临床试验中并没有显示出明显的治疗效果。3.多表位点肽疫苗：多表位点肽疫苗是一种基于前列腺癌相关抗原（PCA）和前列腺特异性抗原（PSA）等多个抗原的肽片段构建而成的免疫治疗方法。它的优点是可以激发多个免疫反应通路，但在临床试验中并没有显示出明显的治疗效果。4.Sipuleucel-T：sipuleucel-T是一种个体化的抗原特异性细胞疫苗，它是唯一被FDA批准用于mCRPC患者的免疫治疗方法。它的优点是可以根据患者的个体情况进行定制化治疗，并且在临床试验中显示出可以延长患者的生存期。但是，它需要多次注射，并且在治疗过程中可能会出现一些不良反应。5.PROSTVAC：PROSTVAC是一种基于天然杀伤细胞（NK细胞）和T细胞免疫反应的免疫治疗方法。它的优点是安全性高，并且在早期临床试验中显示出一定的抗肿瘤活性。但在后期临床试验中并没有显示出明显的治疗效果。表2 评估前列腺癌患者抗肿瘤疫苗的III期试验未来趋势——联合治疗由于肿瘤相关耐药和免疫逃避的多种机制(图2)，前列腺癌疫苗的最佳使用（以及未来5-10年疫苗试验的重点）可能会与针对这些耐药机制的药物联合使用。图2 前列腺癌抗肿瘤疫苗耐药机制及潜在联合治疗方法在文章中，作者提到了联合疗法作为一种新型治疗方式，并讨论了其应用和前景，例如，将免疫治疗与放疗、化疗等传统治疗方法相结合，可以达到协同作用的效果：通过将质粒DNA疫苗与PD1阻断剂联合使用，已经观察到疫苗抗肿瘤活性的增加，这种方法正在几项试验中进一步研究。此外，作者还提到了一些新型联合治疗方法的发展趋势，例如将免疫抑制剂与其他免疫治疗方法相结合，以及使用基因编辑和改造技术进行个体化定制化治疗等。这些新型联合治疗方法有望在未来成为前列腺癌治疗的重要手段。参考文献：1.Rastogi, I et al. “Vaccines as treatments for prostate cancer.” Nature Reviews Urology.
