Interference with the secretion of tears by the lacrimal glands. Obstruction of the LACRIMAL SAC or NASOLACRIMAL DUCT causing acute or chronic inflammation of the lacrimal sac (DACRYOCYSTITIS). It is caused also in infants by failure of the nasolacrimal duct to open into the inferior meatus and occurs about the third week of life. In adults occlusion may occur spontaneously or after injury or nasal disease. (Newell, Ophthalmology: Principles and Concepts, 7th ed, p250)

The Efficacy of Dexmedetomidine Versus Labetalol In Providing Controlled Hypotension In Dacryocystorhinostomy SurgeryA Comparative Randomized Prospective Study

The most important symptoms of nasolacrimal duct obstruction are excessive tearing and mucoid discharge.
The proposed treatment in this regard is dacryocystorhinostomy (DCR) surgery. Therefore, hemostasis is of great significance in performing DCR surgery. In this regard, the reduction of controlled hypotension and the control of hemodynamic responses of the body to stress effectively reduce the bleeding volume during surgery.
Hence, there is no enough studies about controlled hypotension in DCR we decided to perform such a comparison between the effect of dexmedetomidine and labetalol in providing controlled hypotension during DCR surgery.

Evaluation of dimensional changes of the nasolacrimal canal after rapid maxillary expansion: A cone-beam computed tomography study

OBJECTIVEThe aim of this study is to evaluate the nasolacrimal canal dimensionally and volumetrically after treatment in patients who underwent rapid maxillary expansion.MATERIALS AND METHODSA total of 19 patients (12 females, 7 males) with maxillary transverse deficiency, who underwent rapid maxillary expansion were included in the study. Cone beam computed tomography images were analyzed to measure the volume, anteroposterior diameter, transverse diameter, length, and angle of the nasolacrimal duct before and after treatment.RESULTSThe average age of the patients was 13.8 ± 1.69 years. Statistically significant increases in the volume, anteroposterior diameter, transverse diameter, length, and angle of the nasolacrimal canal were observed after treatment for both the right and left sides. No significant difference was found nasolacrimal canal dimensionally and volumetrically except angle on the left and right side of male and female patients. No significant differences were found in the changes between male and female patients.CONCLUSIONRapid maxillary expansion resulted in significant dimensional and volumetric increases in the nasolacrimal canal. Although no prior studies have linked maxillary transverse deficiency to nasolacrimal canal obstruction, the findings suggest that RME may be a viable treatment for nasolacrimal canal obstruction under appropriate conditions.

Effects of mixed exposure to PFAS on adolescent non-alcoholic fatty liver disease: Integrating evidence from human cohorts, toxicogenomics, and animal models to uncover mechanisms and potential target sites

Extensive evidence suggests a correlation between environmental pollutants, specifically perfluoroalkyl and polyfluoroalkyl substances (PFAS) and non-alcoholic fatty liver disease (NAFLD). This study aims to investigate the association and underlying mechanisms of PFAS-induced NAFLD in adolescents by employing a comprehensive approach of population-based studies, toxicogenomics, and animal models. A total of 2014 freshmen from Dali University were recruited for this study, with 1694 participants undergoing serum testing for PFAS exposure. Additionally, Comparative Toxicogenomics Database analysis and PFAS exposure experiments were conducted by orally administering PFAS to 8-week-old adult C57/6 J mice for 28 days. Epidemiological analysis of the adolescent cohort revealed that perfluorohexanesulfonic acid and perfluorooctanoic acid are significant risk factors for NAFLD in adolescents. Toxicogenomic analysis revealed that the negative regulation of gap junction assembly and glutathione derivative biosynthesis contributes to NAFLD development. Animal model studies further demonstrated that combined PFAS exposure led to pathological changes in hepatocytes, including inflammation and steatosis, elevated liver enzymes, cholestasis, and bile canalicular blockage. This study reveals that PFAS exposure serves as a significant risk factor for hepatic steatosis/NAFLD in adolescents. The activation of cytochrome P4502E1 and glutathione S-transferase A1 signaling highlights new molecular targets for PFAS-induced disruptions in hepatic lipid metabolism.

Correlation of the fluorescein dye disappearance test and endoscopic dye transit time in patients after surgery for nasolacrimal duct obstruction

PURPOSETo correlate fluorescein dye disappearance test (FDDT) grades and endoscopic dye transit times (EDTT) in patients with patent osteotomies after nasolacrimal duct obstruction surgery (NLDO).DESIGNCross-sectional study.METHODSAll adult patients with patent osteotomies post-external dacryocystorhinostomy (Ex-DCR) or post-endoscopic dacryocystorhinostomy (EDCR) for NLDO were included in this cross-sectional investigation. Patients with dry eye and endoscopically unidentified osteotomy sites were excluded from the study. The FDDT grade, EDTT and lacrimal symptom questionnaire (Lac-Q) scores were documented for all patients.RESULTSThis study included 39 patients. Nineteen had undergone EDCR, twenty Ex-DCR. The FDDT grade and EDTT were positively correlated (r=0.32, P=0.045). The mean Lac-Q score was not correlated with FDDT grade (r=-0.01, P=0.951) or EDTT (r=0.07, P=0.669). There were no significant differences in the FDDT grade, EDTT, Lac-Q score or ostial characteristics between Ex-DCR and EDCR.CONCLUSIONFDDT grading correlates with EDTT, suggesting that these tests may be used interchangeably based on the clinician's ease of access and instrumentation. There was no correlation between the symptoms of lacrimal outflow obstruction with objective evidence of drainage.
