一项最新的临床研究揭示,在2型糖尿病患者中,血液中的脂蛋白(a)水平升高,可能与一种常见的外周血管并发症——外周动脉疾病的风险增加存在显著关联。
该研究发表在学术期刊British journal of hospital medicine上,题为A Significant Correlation Between Lipoprotein(a) Levels and Peripheral Arterial Disease in Patients With Type 2 Diabetes Mellitus: A Retrospective Study。研究由Chen Chao和Lv Fang共同完成。
为了探究这一关系,研究团队在合肥市第一人民医院进行了一项回顾性分析。研究纳入了从2022年1月到2024年8月期间住院的590名确诊为2型糖尿病的患者作为研究对象。研究人员根据患者入院时检测的脂蛋白(a)浓度,将所有参与者分为低、中、高三个水平组。外周动脉疾病的诊断依据是国际通用的踝臂指数测量,当该指数低于0.9时,即被判定为存在外周动脉疾病。
数据分析结果显示,不同脂蛋白(a)水平组间的外周动脉疾病患病率存在显著差异。具体而言,脂蛋白(a)水平最高的一组患者,其外周动脉疾病的患病率明显高于水平较低的患者组。这一差异具有统计学意义。
在调整了年龄、性别、血压、血脂、血糖控制水平等多种可能混杂因素的影响后,多因素逻辑回归模型的分析结果更加明确了这种关联的强度。与脂蛋白(a)水平最低的一组相比,水平最高的一组患者发生外周动脉疾病的风险增加了约96%,其风险比值达到1.961。此外,分析还发现,脂蛋白(a)水平每升高一个标准差,患病风险相应增加25.7%;而经过对数转换后的脂蛋白(a)每增加一个单位,风险更是上升了80.3%。
研究还通过亚组分析检验了这种关联在不同患者群体中的稳定性。结果表明,无论患者的性别、年龄或是否合并高血压等因素如何,脂蛋白(a)水平与外周动脉疾病风险之间的正相关关系均普遍存在。为了评估脂蛋白(a)作为预测指标的潜力,研究者绘制了受试者工作特征曲线,计算出的曲线下面积为0.622,这表明脂蛋白(a)对于预测2型糖尿病患者是否患有外周动脉疾病具有一定的鉴别能力。
为了更精细地描述两者之间的关系形态,研究者采用了限制性立方样条法进行分析。结果并未发现脂蛋白(a)水平与外周动脉疾病风险之间存在非线性关系的证据,这提示风险可能随着脂蛋白(a)水平的升高而呈现稳定的、连续性的增加趋势。
综上所述,这项回顾性研究提供了证据,表明在2型糖尿病人群中,较高的脂蛋白(a)水平与较高的外周动脉疾病患病风险显著相关。这一发现为临床识别和管理2型糖尿病患者的血管并发症风险提供了一个新的潜在生物标志物视角。

近日,一项题为Genetically Predicted Levels of Lipoprotein(a) and Risk of Cerebrovascular Disease的研究在 Journal of the American Heart Association 期刊上提前在线发表。该研究由 Iyas Daghlas、Marios K. Georgakis 等学者共同完成,旨在利用遗传学方法厘清脂蛋白(a)[Lp(a)]与不同亚型脑血管疾病之间的因果关系。
Lp(a) 是血液中的一种特殊颗粒,已被证实与冠心病风险升高密切相关,是当前降脂药物研发的新热点。然而,其对不同类型脑卒中风险的影响尚未完全明确。为了克服观察性研究中混杂因素和反向因果关系的干扰,本研究采用了孟德尔随机化这一方法。该方法利用与Lp(a)水平密切相关的遗传变异作为“自然随机化”的工具,能够更可靠地推断暴露与结局之间的因果关联。
研究人员选取了两个能强效影响Lp(a)水平的 _LPA_ 基因变异(rs10455872和rs3798220)作为工具,这两个变异共同解释了人体Lp(a)水平36%的个体差异。通过分析大规模遗传学联盟提供的汇总数据,研究者评估了遗传预测的Lp(a)水平(每升高100 nmol/L)与多种脑血管结局之间的关联。
研究的主要发现清晰地指向了Lp(a)致病作用的特异性路径。结果显示,遗传预测的较高Lp(a)水平会显著增加所有缺血性卒中的总体风险。当深入分析缺血性卒中的不同亚型时,关联性出现了明显分化。其中,与大动脉粥样硬化性卒中的关联最为强烈,风险增加了23%。相比之下,与小血管闭塞性(腔隙性)卒中则未发现显著关联。与心源性栓塞性卒中存在较弱的正向关联。
此外,在针对早发性(通常指≤60岁)卒中的分析中,研究者观察到了相似的模式,且Lp(a)与大动脉粥样硬化性卒中的关联强度进一步增大,风险升高了37%。这一发现提示,Lp(a)在年轻人群的动脉粥样硬化性卒中发病中可能扮演着尤为重要的角色。
为了进一步验证和拓展上述发现,研究者还分析了一系列与卒中相关的中间表型。结果同样呈现出具有一致性的模式:遗传预测的高Lp(a)水平与颈动脉斑块的存在显著相关,这直接支持了其促进动脉粥样硬化的作用。同时,研究也发现其与心房颤动风险增加相关,这可能部分解释了其与心源性栓塞性卒中的微弱关联。另一方面,与脑小血管病相关的多个影像学标志物(如白质高信号、腔隙)均未显示出显著关联。
综上所述,这项孟德尔随机化研究提供了强有力的遗传学证据,表明Lp(a)升高是颅内大动脉粥样硬化性缺血性卒中的一个因果风险因素,而其与脑小血管病性卒中无因果关系。这些发现对于未来针对Lp(a)的干预性临床试验具有明确的指导意义,提示应优先考虑将患有动脉粥样硬化性脑血管疾病的个体纳入研究,以评估降低Lp(a)水平对预防此类卒中的潜在益处。

代谢综合征是一组以中心性肥胖、高血糖、血脂异常和高血压为特征的复杂临床症候群,其全球患病率不断上升,寻找安全有效的辅助管理策略成为临床关注的重点。近日,一项发表在Frontiers in nutrition期刊上的系统评价和荟萃分析,评估了传统香料孜然(Cuminum cyminum L.)作为膳食补充剂,对成人代谢紊乱患者代谢综合征关键指标的影响,为该领域提供了新的循证见解。
这项研究由Man Liu、Songze Wu和Maryam Falahatzadeh共同完成,系统检索了截至2025年4月的多个主流医学数据库,最终纳入了9项在伊朗开展的随机对照试验,共涉及531例参与者。研究采用严谨的GRADE方法对证据质量进行评估,旨在综合评价孜然补充剂对空腹血糖、血脂和腰围等核心代谢参数的作用。
荟萃分析结果显示,与安慰剂或对照组相比,补充孜然带来了具有统计学意义的积极变化。具体而言,**孜然补充剂显著降低了参与者的空腹血糖水平**。同时,在血脂谱方面,**孜然显著降低了甘油三酯水平,并提高了对心血管有保护作用的高密度脂蛋白胆固醇水平**。此外,**补充孜然还与腰围的显著减小相关**,这提示其对中心性肥胖可能有改善作用。
研究者进一步通过亚组分析探索了可能影响效果的变量。分析发现,在**年龄超过50岁的参与者**以及**被诊断为2型糖尿病的患者**中,孜然在降低空腹血糖和甘油三酯方面的益处似乎更为突出。有趣的是,**较低剂量的孜然补充(每日低于100毫克)** 在改善空腹血糖和升高高密度脂蛋白胆固醇方面显示出效果。大多数有益效应在**干预持续时间超过8周**的研究中更为明显。

综上所述,这项荟萃分析表明,孜然补充剂可能作为改善成人代谢综合征患者部分代谢指标的潜在辅助手段。然而,要得出确切的临床推荐结论,未来仍需在不同地区、不同人群中进行更多设计严谨、样本量更大、干预周期更长的随机对照试验予以验证。

此外,脂质与免疫的互动战场也延伸到了感染领域。狡猾的病原体,如某些病毒和细菌,进化出了劫持宿主细胞脂质代谢通路的能力。它们通过改变宿主细胞的脂质环境,为自己创造有利的生存空间,并借此削弱或逃避免疫系统的识别与攻击。这一发现揭示了感染性疾病中免疫逃逸的新机制。
