Predominant cell surface sialoprotein of leukocytes which regulates multiple T-cell functions, including T-cell activation, proliferation, differentiation, trafficking and migration. Positively regulates T-cell trafficking to lymph-nodes via its association with ERM proteins (EZR, RDX and MSN) (By similarity). Negatively regulates Th2 cell differentiation and predisposes the differentiation of T-cells towards a Th1 lineage commitment. Promotes the expression of IFN-gamma by T-cells during T-cell receptor (TCR) activation of naive cells and induces the expression of IFN-gamma by CD4(+) T-cells and to a lesser extent by CD8(+) T-cells (PubMed:18036228). Plays a role in preparing T-cells for cytokine sensing and differentiation into effector cells by inducing the expression of cytokine receptors IFNGR and IL4R, promoting IFNGR and IL4R signaling and by mediating the clustering of IFNGR with TCR (PubMed:24328034). Acts as a major E-selectin ligand responsible for Th17 cell rolling on activated vasculature and recruitment during inflammation. Mediates Th17 cells, but not Th1 cells, adhesion to E-selectin. Acts as a T-cell counter-receptor for SIGLEC1 (By similarity). Protects cells from apoptotic signals, promoting cell survival.

[Clinical Significance of Genetic and Molecular Changes in Primary Myeloid Sarcoma].

OBJECTIVE:

To investigate the clinical significance of genetic and molecular changes in primary myeloid sarcoma (MS).

METHODS:

Fourteen patients with primary MS were selected in Jiading District Central Hospital Affiliated Shanghai University of Medicine & Health Sciences, The First People's Hospital of Lianyungang from September 2010 to December 2021. AML1-ETO fusion, PML-RARα fusion and CBFβ breakage were detected by fluorescence in situ hybridization (FISH), and the mutations of NPM1, CEBPA, FLT3, RUNX1, ASXL1, KIT and TP53 genes were detected by new generation sequencing (NGS).

RESULTS:

Among 14 patients, the MS occurred in bone, breast, epididymis, lung, chest wall, cervix, small intestine, ovary, lymph nodes and central nervous system. The tumor cells expressed MPO (13 cases), CD34 (7 cases), CD43 (8 cases), CD68 (7 cases), CD99 (8 cases) and CD117 (6 cases). Cytogenetic abnormalities were observed in 4 cases, including 3 cases of AML1-ETO fusion and 1 case of CBFβ breakage, while no PML-RARα fusion was detected. There were no significant differences in overall survival (OS) and leukemia-free survival (LFS) between patients with and without AML1-ETO fusion/CBFβ breakage (both P >0.05). Among the 14 patients, the number of NPM1, CEBPA, FLT3-ITD, RUNX1, ASXL1, KIT and TP53 gene mutations was 5, 3, 5, 3, 2, 2, 1, respectively, of which 7 cases had at least one mutation in FLT3-ITD, RUNX1, ASXL1 and TP53 gene. The OS and LFS of patients with FLT3-ITD, RUNX1, ASXL1 or TP53 mutation were shorter than those without mutations (both P <0.01).

CONCLUSION:

The genetic and molecular abnormalities of primary MS can be detected by FISH and NGS techniques. FLT3-ITD, RUNX1, ASXL1 or TP53 mutation indicates a worse prognosis, but further clinical studies are needed to confirm it.

Prediction model of radiotherapy outcome for Ocular Adnexal Lymphoma using informative features selected by chemometric algorithms.

BACKGROUND:

Ocular Adnexal Lymphoma (OAL) is a non-Hodgkin's lymphoma that most often appears in the tissues near the eye, and radiotherapy is the currently preferred treatment. There has been a controversy regarding the prognostic factors for systemic failure of OAL radiotherapy, the thorough evaluation prior to receiving radiotherapy is highly recommended to better the patient's prognosis and minimize the likelihood of any adverse effects.

PURPOSE:

To investigate the risk factors that contribute to incomplete remission in OAL radiotherapy and to establish a hybrid model for predicting the radiotherapy outcomes in OAL patients.

METHODS:

A retrospective chart review was performed for 87 consecutive patients with OAL who received radiotherapy between Feb 2011 and August 2022 in our center. Seven image features, derived from MRI sequences, were integrated with 122 clinical features to form comprehensive patient feature sets. Chemometric algorithms were then employed to distill highly informative features from these sets. Based on these refined features, SVM and XGBoost classifiers were performed to classify the effect of radiotherapy.

RESULTS:

The clinical records of from 87 OAL patients (median age: 60 months, IQR: 52-68 months; 62.1% male) treated with radiotherapy were reviewed. Analysis of Lasso (AUC = 0.75, 95% CI: 0.72-0.77) and Random Forest (AUC = 0.67, 95% CI: 0.62-0.70) algorithms revealed four potential features, resulting in an intersection AUC of 0.80 (95% CI: 0.75-0.82). Logistic Regression (AUC = 0.75, 95% CI: 0.72-0.77) identified two features. Furthermore, the integration of chemometric methods such as CARS (AUC = 0.66, 95% CI: 0.62-0.72), UVE (AUC = 0.71, 95% CI: 0.66-0.75), and GA (AUC = 0.65, 95% CI: 0.60-0.69) highlighted six features in total, with an intersection AUC of 0.82 (95% CI: 0.78-0.83). These features included enophthalmos, diplopia, tenderness, elevated ALT count, HBsAg positivity, and CD43 positivity in immunohistochemical tests.

CONCLUSION:

The findings suggest the effectiveness of chemometric algorithms in pinpointing OAL risk factors, and the prediction model we proposed shows promise in helping clinicians identify OAL patients likely to achieve complete remission via radiotherapy. Notably, patients with a history of exophthalmos, diplopia, tenderness, elevated ALT levels, HBsAg positivity, and CD43 positivity are less likely to attain complete remission after radiotherapy. These insights offer more targeted management strategies for OAL patients. The developed model is accessible online at: https://lzz.testop.top/.

CNS erythroblastic sarcoma: a potential emerging pediatric tumor type characterized by NFIA::RUNX1T1/3 fusions.

Erythroblastic sarcoma (ES) (previously called chloroma or granulocytic sarcoma) are rare hematological neoplams characterized by the proliferation of myeloid blasts at extramedullary sites, and primarily involve the skin and soft tissue of middle-aged adults. ES may be concomitant with or secondary to myeloid neoplasms (mostly acute myeloid leukemia (AML)) or in isolated cases (de novo) without infiltration of the bone marrow by blasts. ES share cytogenetic and molecular abnormalities with AML, including RUNX1T1 fusions. Some of these alterations seem to be correlated with particular sites of involvement. Herein, we report an isolated erythroblastic sarcoma with NFIA::RUNX1T1 located in the central nervous system (CNS) of a 3-year-old boy. Recently, two pediatric cases of CNS MS with complete molecular characterization have been documented. Like the current case, they concerned infants (2 and 3 years-old) presenting a brain tumor (pineal involvement) with leptomeningeal dissemination. Both cases also harbored a NFIA::RUNX1T3 fusion. ES constitutes a diagnostic challenge for neuropathologists because it does not express differentiation markers such as CD45, and may express CD99 which could be confused with CNS Ewing sarcoma. CD43 is the earliest pan-hematopoietic marker and CD45 is not expressed by erythroid lineage cells. E-cadherin (also a marker of erythroid precursors) and CD117 (expressed on the surface of erythroid lineage cells) constitute other immunhistochemical hallmarks of ES. The prognosis of patients with ES is similar to that of other patients with AML but de novo forms seem to have a poorer prognosis, like the current case. To conclude, pediatric ES with NFIA::RUNX1T1/3 fusions seem to have a tropism for the CNS and thus constitute a potential pitfall for neuropathologists. Due to the absence of circulating blasts and a DNA-methylation signature, the diagnosis must currently be made by highlighting the translocation and expression of erythroid markers.

Poster one, entitled " KBA1413, an antibody derived from a cured AML patient, recognizes a unique CD43 glycoform shared by AML, MDS and solid cancer cells," details the identification of KBA1413, a fully human antibody screened from a single acute myeloid leukemia (AML) patient who remains in long term remission following allogeneic hematopoietic stem cell transplantation. The antibody identified using Kling's proprietary B cell screening platform, Kling-Select, recognised a unique, previously undescribed, sialylated epitope on CD43 that is reactive to freshly isolated blasts of over 60 randomly selected AML and myelodysplastic syndrome (MDS) patients. It also recognised CD43 on several solid tumor indications, including melanoma and breast cancer. KBA1413 triggered TR NK-mediated antibody dependent cell-mediated cytotoxicity (ADCC) against AML cells both in vitro and in vivo, suggesting that the antibody played a role in the graft versus leukemia response of the original donor patient. KBA1413 was made into a bispecific T-cell engaging antibody (bTCE), to increase its therapeutic potential and induced potent cytotoxicity in vitro and in vivo on cell lines in primary AML. Data demonstrates that KBA1413 warrants further development as a potent and promising treatment candidate for AML, MDS and solid tumors. The Kling-Select platform was used to discover and characterise anti-snRNP200 antibodies from three AML patients that have achieved complete response after allogeneic stem cell transplantation, establishing that anti-snRNP200 specifically and selectively bind to AML cell lines that display snRNP200 on the outer cell surface. These findings have been independently confirmed[2] and show that snRNP200 surface expression is limited to malignant cells and not expressed on normal hematopoietic stem cell progenitors. Kling will further explore the therapeutic potential of anti-snRNP200 with affinity matured variants produced with Kling-Evolve. 近日，美国西北大学的研究团队获得了精神病神经基础理解的里程碑式发现——与抗精神病药物相结合调控纹状体输出并不是D2R，而是D1R。精神分裂症是一种严重的精神障碍，患者可能出现感知、思维和情感方面的严重紊乱，对日常生活产生十分恶劣的影响。多巴胺假说是精神分裂症发病机制的重要假说之一。多巴胺假说认为，精神分裂症可能与大脑中多巴胺的过度活跃有关。过度活跃的神经递质导致特定神经回路的失衡，从而引发幻觉、妄想、思维紊乱、情感疏离等症状。抑制多巴胺活性被认为是抗精神病药物的主要作用机制之一。大脑纹状体区域的棘状投射神经元（SPNs）分别表达多巴胺受体D1（D1R）和多巴胺受体D2（D2R），受体与Gαs偶联可以增加或降低SPN活动。通过调节SPN，可以达到平衡纹状体输出的目的。以往科学家认为，抗精神病药物是通过与D2R结合来抑制多巴胺活性的。事实上，D2R在纹状体之外的脑细胞中也有表达，根据受体表达水平预测的药物治疗效果往往与实际存在误差甚至完全相反。近日，美国西北大学的研究团队获得了精神病神经基础理解的里程碑式发现——与抗精神病药物相结合调控纹状体输出并不是D2R，而是D1R。研究发现，有些药物虽然能够使D2-SPN动态正常化，但是却没有产生临床效果。药物的抗精神病功效与D1-SPN的动力学逆转有关。选择性抑制D1-SPN可以减弱安非他命驱动的过度运动、幻觉等症状。研究发表在《自然·神经科学》上。研究人员通过神经群体成像方法记录小鼠大脑D1-SPN和D2-SPN的活动。使用安非他命诱导多巴胺释放，神经元活动与之前观察的一致，D1-SPN活动水平增加，D2-SPN活动水平降低。接下来，研究人员选择了4种抗精神病药物，评估其对神经元活动的影响。其中，氟哌啶醇有显著的运动障碍副作用，氯氮平和奥氮平运动障碍副作用较轻，MP-10为临床失败的精神分裂症候选药物。根据小鼠用药后的运动能力，设置高、低两种药物剂量。结果显示，3种临床有效药物均可以减轻安非他命引起的D1-SPN神经元动态紊乱、过度运动和感觉运动门控异常（即感觉和运动之间的调节失常）。相比之下，MP-10虽然减轻了安非他命引起的D2-SPN活动水平降低，但是加剧了D1-SPN的过度活动，且并未纠正小鼠的感觉运动门控异常。抗精神病药物改善小鼠的过度运动这样的结果提示我们，是不是对D1-SPN活动的纠正，才是药物起效的关键呢？研究人员用其他新型抗精神病药物候选化合物进行了测试。虽然这些化合物的作用机制并不是与多巴胺受体结合，但所有产生临床疗效的化合物，都可以减轻安非他命引起的D1-SPN过度活动。而通过化学方法抑制D1-SPN活性，也可以减轻安非他命导致的小鼠幻听。药物对D1-SPN的影响能更好地解释抗精神病药物的疗效分别使用D1R部分激动剂SKF38393、D1R拮抗剂SCH23390/SCH39166处理小鼠，结果显示，3种化合物可以减轻安非他命引起的过度运动和感觉运动门控异常。其中，SKF38393在基线条件下促进D1-SPN活动，在多巴胺过度释放的条件下抑制D1-SPN活动，表现出多巴胺状态依赖性的D1-SPN选择性抑制。总的来说，研究证明，抗精神病药物的疗效与其恢复D1-SPN的正常活动有关，为开发副作用更小的药物开辟了新的选择。研究人员表示，“这是一个里程碑式发现，彻底改变了我们对精神病神经基础的理解”。
