Confluent and reticulated papillomatosis (CARP) presents with hyperpigmented macules and papules in a net-like distribution, usually confluent over the central chest, back, and neck, and reticulated as it spreads out centrifugally over the outer trunk and upper extremities.The authors conducted an institution review board e approved retrospective chart review between 2009 and 2020 within the Mount Sinai Health System, and medical records of patients identified through a database search for CARP diagnoses were reviewed for demographics, clin. findings, treatments, and treatment response.Treatments included systemic antibiotics (minocycline, doxycycline, and azithromycin), topical antibiotics, and other topicals, including ammonium lactate and corticosteroids.CARP is common not only in adolescents, as previously described, but also in adults, and associated with obesity (48%), which was noted with more than twice the frequency than the 22% in New York City.Minocycline is the treatment of choice for CARP, and clin. response can confirm the clin. diagnosis.In this study, CARP treated with doxycycline 100 mg twice a day had similar response and lower recurrence rates than minocycline 100 mg twice a day but a longer course of treatment.To our knowledge, this study describes the largest cohort of patients with CARP to date, which supports using doxycycline or minocycline for CARP management.Patients with CARP were more likely to be overweight or obese.
