(Microbial infection) Acts as a co-receptor for hepatitis C virus (HCV) in hepatocytes (PubMed:17325668, PubMed:20375010, PubMed:24038151). Associates with CD81 and the CLDN1-CD81 receptor complex is essential for HCV entry into host cell (PubMed:20375010). Acts as a receptor for dengue virus (PubMed:24074594). Claudins function as major constituents of the tight junction complexes that regulate the permeability of epithelia. While some claudin family members play essential roles in the formation of impermeable barriers, others mediate the permeability to ions and small molecules. Often, several claudin family members are coexpressed and interact with each other, and this determines the overall permeability. CLDN1 is required to prevent the paracellular diffusion of small molecules through tight junctions in the epidermis and is required for the normal barrier function of the skin. Required for normal water homeostasis and to prevent excessive water loss through the skin, probably via an indirect effect on the expression levels of other proteins, since CLDN1 itself seems to be dispensable for water barrier formation in keratinocyte tight junctions (PubMed:23407391).

A Randomized, Double-Blind, Placebo-Controlled Study of Intravenously Administered ALE.F02 to Evaluate the Safety, Tolerability, Pharmacokinetics, and Renal Sparing in Antineutrophil Cytoplasmic Antibody-Associated Vasculitis With Rapidly Progressive Glomerulonephritis

The goal of this clinical trial is to learn if a new drug that might help protect and preserve kidney function in antineutrophil cytoplasmic antibody (ANCA) associated vasculitis (AAV). AAV is a type of autoimmune disease where the body's own immune system attacks itself, and in the case of AAV the body attacks its own small blood vessels. There are many small blood vessels in the kidneys meaning the kidneys are commonly affected in AAV.
The main questions it aims to answer are:
Is the new drug well tolerated and safe?
Can the new drug protect and preserve kidney functions when is added to standard therapy?
Researchers will compare the following groups to see how the new drug is tolerated and what effect to preserve kidney tissue has:
Group A: Standard treatment + ALE.F02 lower dose infusions
Group B: Standard treatment + ALE.F02 higher dose infusions
Group C: Standard treatment + placebo infusions (inactive substance)
The Treatment period will consist of 24 weeks beginning on Day 1, during which time participants will receive 13 infusions of the study medicine, along with standard therapy for kidney inflammation due to AAV. During the treatment period, participants will have the following assessments:
A brief physical examination focusing on their skin any pre-existing medical conditions that you have.
Collection of blood and urine samples for routine safety tests and to assess renal function.
Collection of blood samples:
To measure the amount of study medicine in their blood. This is called pharmacokinetics (PK) and it is tested to see how study medicine enters, moves through, and exits the body.
To test for antidrug antibodies (ADA). To check if their body create antibodies against the study medicine, as this could reduce its effect.
To measure biomarkers. Biomarkers are specific compounds in the body (can be protein, hormones, or genetic molecules) that indicate normal or abnormal processes taking place in your body and may be a sign of an underlying condition or disease (for example glucose levels are used as biomarker in managing diabetes). They are used to see how well the body responds to a treatment for a disease or condition.
Collection of urine to measure urine markers of vasculitis/inflammation called biomarkers.
Urine pregnancy test. A urine pregnancy test is a quick medical test that can tell if a woman is pregnant or not by checking for a hormone which is produced during pregnancy, usually in the urine.
Chest High Resolution Computed Tomography (HRCT) scan to check whether they have vasculitis affecting their lungs. A CT scan uses special x-ray equipment to take detailed pictures of body tissues and organs to diagnose and monitor conditions in various parts of the body. For the CT scan, they will need to lie still on a table. At Week 24 a second lung CT scan will be performed for participants whose initial scan showed lung vasculitis to see whether your lung vasculitis is getting better or ongoing/worse.

Design and validation of a custom-made system to measure transepithelial electrical impedance in human corneas preserved in active storage machine.

Corneal epithelial barrier represents one of the major limitations to ocular drug delivery and can be explored non-invasively through the evaluation of its electrical properties. Human corneas stored in active storage machine (ASM) could represent an interesting physiological model to explore transcorneal drug penetration. We designed a new system adapted to human corneas preserved in ASM to explore corneal epithelial barrier function ex-vivo. A bipolar set-up including Ag/AgCl electrodes adaptors to fit the corneal ASM and a dedicated software was designed and tested on freshly excised porcine corneas (n = 59) and human corneas stored 14 days in ASM (n = 6). Porcine corneas presented significant and proportional decrease in corneal impedance in response to increasing-size epithelial ulcerations and acute exposure to benzalkonium chloride (BAC) 0.01 and 0.05%. Human corneas stored 14 days in ASM presented a significant increase in corneal impedance associated with the restoration of a multi-layer epithelium and an enhanced expression of tight junctions markers zonula occludens 1, claudin 1 and occludin. These results support the relevance of the developed approach to pursue the exploration and development of human corneas stored in ASM as a physiological pharmacological model.

Aged oolong tea alleviates dextran sulfate sodium-induced colitis in mice by modulating the gut microbiota and its metabolites

In this study, the mechanism of aged oolong tea (AOT) to alleviate colitis was investigated in terms of microbiome, metabolome, and fecal microbiota transplantation (FMT). AOT storage period could alleviate colitis in mice and there were some differences in AOT between storage periods, especially AOT-10. AOT improves UC by modulating oxidative stress and inflammatory factors and upregulating intestinal tight junction protein expression (Occludin, Claudin-1, ZO-1 and MUC2), which is associated with the recovery of gut microbiota. FMT and targeted metabolomics further demonstrate that the anti-inflammatory effects of AOT can reshape the gut microbiota through faecal bacterial transfer. Anti-inflammatory effects are exerted through the stimulation of metabolic pathways associated with amino acid, fatty acid and bile acid metabolites. Importantly, the study identified key bacteria (e.g., Sutterella, Clostridiaceae_Clostridium, Mucispirillum, Oscillospira and Ruminococcus) for the development and remission of inflammation. Conclusively, AOT may have great potential in the future adjuvant treatment of colitis.

Fucosylated chondroitin sulfate from sea cucumber Stichopus chloronotus alleviate the intestinal barrier injury and oxidative stress damage in vitro and in vivo

This study aimed to investigate the alleviative effects of fucosylated chondroitin sulfate from sea cucumber Stichopus chloronotus (fCSSc) on the intestinal barrier injury and oxidative stress damage in vitro and in vivo. The results showed that fCS-Sc protected the intestinal barrier and improved the antioxidant function in H₂O₂ damaged Caco-2 cells via up-regulating the tight junction proteins and activating Keap1-Nrf2-ARE antioxidant pathway. Furthermore, administration fCS-Sc could ameliorate the weight loss and spleen index decrease in Cyclophosphamide (Cy) treated mice, improve the expressions of ZO-1, Claudin-1, Nrf2, SOD, and NQO-1 in Cy damaged colon tissue, showing significant protective effects against intestinal barrier damage and oxidative stress in vivo. fCS-Sc intervention also alleviated the gut microbiota disorder though increasing the richness and diversity of intestinal bacteria, regulating the structural composition of gut microbiota. fCS-Sc promoted the relative abundance of beneficial microbiota and inhibited the growth of harmful bacteria. This study provided a theoretical basis for the application of fCS-Sc as a prebiotic in chemotherapy.

Manufacturing Issues Block Astellas From a First-in-Class Cancer Drug Approval

Astellas Pharma’s path to winning the first drug approval for a particular promising gastrointestinal cancer target has hit a detour. The FDA turned down the drugmaker’s application, citing manufacturing issues for the therapy, zolbetuximab. Astellas announced the FDA action earlier this week. The company did not go into detail about the problems spotted by the regulator, but described them as “unresolved deficiencies following its pre-license inspection of a third-party manufacturing facility for zolbetuximab.” Astellas added that the agency did not raise any concerns about the safety or efficacy of the drug, nor is it asking for additional clinical data. Zolbetuximab was developed as a treatment for patients with locally advanced or metastatic gastric or gastroesophageal junction (GEJ) adenocarcinoma that is negative for the cancer protein HER2. Cancers that express HER2 already have treatments, such as Herceptin from Genentech and Enhertu from partners AstraZeneca and Daiichi Sankyo. HER2-negative cancers have fewer treatment options. Astellas aims to offer one with zolbetuximab, which targets claudin 18.2, or CLDN18.2. This protein is found only in stomach cells and not in any other healthy tissues. That’s what makes it a promising target for drugs that treat gut cancers. The Astellas drug is a monoclonal antibody designed to target and bind to CLDN18.2 on the surface of cancerous gastric epithelial cells. Doing so is intended to kill the cancer cells by activating two different immune system pathways: antibody-dependent cellular cytotoxicity and complement-dependent cytotoxicity. Astellas added zolbetuximab to its pipeline through the 2016 acquisition of Germany-based Ganymede Pharmaceuticals for €422 million (about $462 million) up front. Zolbetuximab is a key piece of Astellas’s growth strategy. It’s important because it could help make up for coming revenue declines facing Xtandi, the drugmaker’s top-selling product that is facing patent expirations. Zolbetuximab is under regulatory review in other markets including Japan, Europe, and China. “We remain confident in zolbetuximab’s clinical profile and potential to fill a significant therapeutic gap for those diagnosed with advanced gastric or GEJ cancer whose tumors are CLDN18.2 positive,” Moitreyee Chatterjee-Kishore, Astellas senior vice president and head of immuno-oncology, said in a prepared statement. “Astellas is committed to working with the FDA and the third-party manufacturer to address the agency’s feedback, and to bringing zolbetuximab to U.S. patients in need, as soon as possible.” Several other companies are also developing CLDN18.2-targeting drugs. AstraZeneca’s early-stage program, licensed from KYM Biosciences last year, is part of the class of cancer therapies called antibody drug conjugates, or ADCs. Bristol Myers Squibb and Elevation Oncology are also in Phase 1 testing with their respective CLDN18.2-targeting ADCs. Merck KGaA holds an option to license a Jiangsu Hengrui Pharmaceuticals ADC for that target under a deal it struck with the China-based biotech last fall. Meanwhile, Alentis Therapeutics has two programs, an antibody and an ADC, each of them targeting the related claudin 1 protein.
