Cell surface proteoglycan (PubMed:14610063). Negatively regulates the hedgehog signaling pathway when attached via the GPI-anchor to the cell surface by competing with the hedgehog receptor PTC1 for binding to hedgehog proteins (By similarity). Binding to the hedgehog protein SHH triggers internalization of the complex by endocytosis and its subsequent lysosomal degradation (By similarity). Positively regulates the canonical Wnt signaling pathway by binding to the Wnt receptor Frizzled and stimulating the binding of the Frizzled receptor to Wnt ligands (PubMed:16227623, PubMed:24496449). Positively regulates the non-canonical Wnt signaling pathway (By similarity). Binds to CD81 which decreases the availability of free CD81 for binding to the transcriptional repressor HHEX, resulting in nuclear translocation of HHEX and transcriptional repression (By similarity). Inhibits the dipeptidyl peptidase activity of DPP4 (PubMed:17549790). Plays a role in limb patterning and skeletal development by controlling the cellular response to BMP4 (By similarity). Interleukin-15 and -21 Armored Glypican-3-specific Chimeric Antigen Receptor Expressing Autologous T Cells as an Immunotherapy for Children With Solid Tumors (CARE)

Patients may be considered if the cancer has come back, has not gone away after standard treatment or the patient cannot receive standard treatment. This research study uses special immune system cells called CARE T cells, a new experimental treatment.
The body has different ways of fighting infection and disease. No single way seems perfect for fighting cancers. This research study combines two different ways of fighting cancer: antibodies and T cells. Antibodies are types of proteins that protect the body from infectious diseases and possibly cancer. T cells, also called T lymphocytes, are special infection-fighting blood cells that can kill other cells, including cells infected with viruses and tumor cells. Both antibodies and T cells have been used to treat patients with cancers. They have shown promise, but have not been strong enough to cure most patients.
Investigators have found from previous research that they can put a new gene (a tiny part of what makes-up DNA and carries a person's traits) into T cells that will make them recognize cancer cells and kill them. In the lab, investigators made several genes called a chimeric antigen receptor (CAR), from an antibody called GPC3. The antibody GPC3 recognizes a protein found solid tumors including pediatric liver cancers. This CAR is called GPC3-CAR. To make this CAR more effective, investigators also added two genes that includes IL15 and IL21, which are protein that helps CAR T cells grow better and stay in the blood longer so that they may kill tumors better. The mixture of GPC3-CAR and IL15 plus IL21 killed tumor cells better in the laboratory when compared with CAR T cells that did not have IL15 plus IL21 .This study will test T cells that investigators made (called genetic engineering) with GPC3-CAR and the IL15 plus IL21 (CARE T cells) in patients with GPC3-positive solid tumors.
T cells made to carry a gene called iCasp9 can be killed when they encounter a specific drug called AP1903. The investigators will insert the iCasp9 and IL15 plus IL21 together into the T cells using a virus that has been made for this study. The drug (AP1903) is an experimental drug that has been tested in humans with no bad side-effects. The investigators will use this drug to kill the T cells if necessary due to side effects.
This study will test T cells genetically engineered with a GPC3-CAR and IL15 plus IL21 (CARE T cells) in patients with GPC3-positive solid tumors.
The CARE T cells are an investigational product not approved by the Food and Drug Administration.
The purpose of this study is to find the biggest dose of CARE T cells that is safe, to see how long they last in the body, to learn what the side effects are and to see if the CARE T cells will help people with GPC3-positive solid tumors.

Retroperitoneal Sarcomatoid Yolk Sac Tumor in a Chemotherapy-Naive Patient With Testicular Postpubertal Type Teratoma: A Rare Case Report With Emphasis on Molecular Features.

Sarcomatoid yolk sac tumor is a very rare histologic type of testicular germ cell tumor and is mainly reported in testicular germ cell tumor patients who receive chemotherapy. Herein, we report an extremely rare concurrent retroperitoneal sarcomatoid yolk sac tumor in a man with a testicular postpuberal teratoma before he received chemotherapy. A 37-year-old man initially presented with a persistent abdominal pain. Subsequent imaging studies revealed a 9.6-cm retroperitoneal mass, and 2 testicular masses (3.1 cm and 0.9 cm in greatest dimension, respectively). His serum tumor markers were within normal ranges. His radical orchiectomy demonstrated a postpubertal type teratoma with an adjacent scarring nodule. Later, his retroperitoneal tumor showed spindle tumor cells embedded in predominantly myxoid and focally fibrous stroma with diffuse and strong immunoreactivity for keratin AE1/AE3, SALL4 and glypican 3. No tumor necrosis or brisk mitotic figures were observed. A diagnosis of sarcomatoid yolk sac tumor was rendered. Fluorescence in situ hybridization analysis of his retroperitoneal sarcomatoid yolk sac tumor revealed polysomy 12 and MYC amplification, whereas no evidence of isochromosome 12p [i(12p)], and DNA sequencing showed 6 mutations per megabase (muts/Mb), and the somatic alterations included ARAF amplification and ATR I774Yfs*5. Considering its rarity, sarcomatoid yolk sac tumor may pose diagnostic challenges. Therefore, relevant clinicoradiologic information and ancillary work up, including immunohistochemistry and molecular studies, may be helpful for the accurate classification. Our tumor further raises awareness of this rare event, expands the spectrum of its clinical presentation, and explores the molecular features.

High serum alpha-fetoprotein and positive immunohistochemistry of alpha-fetoprotein are related to poor prognosis of gastric cancer with liver metastasis.

Liver metastasis in gastric cancer is incurable. Alpha-fetoprotein-producing gastric cancer has a poor prognosis and is prone to liver metastasis. We investigated the association between preoperative serum alpha-fetoprotein levels, liver metastasis, and expression of primitive enterocyte phenotype markers. We reviewed the medical records of 401 patients with gastric cancer who underwent curative surgical resection and immunohistochemically evaluated the primitive phenotype markers. The preoperative serum alpha-fetoprotein levels were elevated and normal in 8 and 393 patients, respectively. Liver metastasis was more frequent in patients with higher preoperative alpha-fetoprotein levels. The 5-year postoperative recurrence-free survival and overall survival rates were significantly worse in patients with higher preoperative serum alpha-fetoprotein levels. Although alpha-fetoprotein and Glypican3 and Spalt-like transcription factor 4 tended to be stained with high preoperative serum alpha-fetoprotein levels, these markers were also positive in some patients with normal alpha-fetoprotein levels. In summary, patients with gastric cancer and high preoperative serum alpha-fetoprotein levels have a poor prognosis and high incidence of liver metastasis. Alpha-fetoprotein can help detect liver metastasis relating to the primitive enterocyte phenotype.

GenX caused liver injury and potential hepatocellular carcinoma of mice via drinking water even at environmental concentration.

Hexafluoropropylene oxide dimer acid (GenX) is an alternative to perfluorooctanoic acid (PFOA), whose environmental concentration is close to its maximum allowable value established by the US Environmental Protection Agency, so its effects on human health are of great concern. The liver is one of the most crucial target organ for GenX, but whether GenX exposure induces liver cancer still unclear. In this research project, male C57 mice were disposed to GenX in drinking water at environmental concentrations (0.1 and 10 μg/L) and higher concentrations (1 and 100 mg/L) for 14 weeks to explore its effects on liver injury and potential carcinogenicity in mice. GenX was found to cause a dose-dependent increase in the serum levels of alanine aminotransferase (ALT), aspartate aminotransferase (AST), total cholesterol (TC), and triglyceride (TG). As the content of GenX in drinking water increased, so did the concentrations of Glypican-3 (GPC-3) and detachment gamma-carboxyprothrombin (DCP), indicators of early hepatocellular cancer. GenX destroyed the boundaries and arrangements of hepatocytes, in which monocyte infiltration, balloon-like transformation, and obvious lipid vacuoles were observed between cells. Following exposure to GenX, Masson sections revealed a significant quantity of collagen deposition in the liver. Alpha-feto protein (AFP), vascular endothelial growth factor (VEGF), Ki67, matrix metalloproteinase 2 (MMP-2) and matrix metalloproteinase 9 (MMP-9) gene expression increased in a dose-dependent manner in the treatment group relative to the control group. In general, drinking water GenX exposure induced liver function impairment, elevated blood lipid level, caused liver pathological structure damage and liver fibrosis lesions, changed the liver inflammatory microenvironment, and increased the concentration of liver-related tumor indicator even in the environmental concentration, suggesting GenX is a potential carcinogen.

CAR-T共表达细胞因子的选择

外源性细胞因子工程化被认为是增强抗肿瘤T细胞免疫的解决方案之一。常见细胞因子对CAR-T细胞的作用包括：细胞因子对CAR-T细胞特性的影响IL-2增强增殖和效应能力IL-7延长潜在寿命，提高效应功能的持久性IL-9维持T细胞干性，增强效应功能IL-15延长细胞寿命IL-8增强T细胞向肿瘤迁移的能力IL-10通过代谢重编程恢复耗竭T细胞功能IL-12直接上调细胞毒性，调节免疫抑制TMEIL-18增强效应功能，重塑TMEIL-23提高效应功能，减少耗竭表型IL-33调节TMEIL-36γ增强细胞毒性，调节TMEIL-37恢复耗竭T细胞功能在实际开发应用中，把握共表达细胞因子的安全性和有效性依然是一个挑战。近期还是有一些相关研究团队继续关注着细胞因子CAR结构的开发。IL-4IL-4是肿瘤细胞在TME中释放的细胞因子，可以通过与T细胞表面的IL-4受体结合来对抗T细胞效应功能。通过转化细胞因子受体CAR(ICR)是抑制这种免疫抑制作用的常见方法，IL-7和IL-21通常是对应转化细胞因子的选择。华师大的研究团队近期则设计了一款4/15NKG2D-CAR，由基于NKG2D的嵌合抗原受体组成，IL-4R作为细胞外结构域共表达，IL-15R作为跨膜和细胞内结构域共表达，从而将IL-4R抑制信号转化为下游的IL-15R激活信号。(doi:10.1016/j.biopha.2023.115740.)4/15NKG2D-CAR-T在小鼠模型中表现出提高细胞毒性、增殖水平、存活率、未分化表型比例等抗肿瘤作用。IL-7(doi:10.1007/s10565-023-09821-w)研究人员设计了一款GPC3-7-19-CAR-T细胞用于治疗HCCs。杀伤效率是单纯GPC3-CAR-T细胞的1.5-2倍。在小鼠实验中表现出提高浸润CD4+ T细胞亚群比例、募集更成熟的DCs、减少PMN-MDSC和Treg细胞浸润，从而诱导TME重塑。该CAR-T在1例GPC3+ 晚期HCC患者身上展开了临床试验。结果显示，CAR-T治疗后，淋巴结节明显缩小，CT扫描显示CAR-T注射后56天患者纵隔淋巴囊萎缩，CD8+ T细胞的比例(67.9%)远高于基线(16.6%)。采用的患者来源CAR-T细胞阳性率为69%。IL-10IL-10在肿瘤发展和发病机制中的作用似乎是自相矛盾的。IL-10本身具有强大的抗肿瘤作用，通过免疫依赖性机制抑制转移，包括抑制浸润性巨噬细胞和血管生成因子以及激活CD8 + T细胞CTL。但同时，研究也报道了TAM产生的IL-10可以通过抑制APC的功能并随后阻断T细胞效应功能，来减弱抗肿瘤反应。矛盾的IL-10(doi:10.1038/s41587-023-02060-8)2024年1月浙江大学/唐力教授团队合作研究开发了一款表达IL-10的CAR-T，在小鼠实验中表现出能够维持线粒体健康、促进OXPHOS代谢等代谢重编程功能，实现肿瘤的消除。据称该CAR-T已经展开IIT临床试验，至少已有11名复发难治性淋巴瘤或复发难治性急性白血病患者达到完全缓解，CR接近100%。重新认识IL-10：提升CAR-T的CR至100%IL-13Rα2IL-13Rα2则和其他细胞因子的使用不同，其主要是因为在肿瘤上特异性表达而被当做靶点。1.(doi:10.1038/s41591-024-02893-z)近期野马生物/希望之城公布了其IL13Rα2靶向CAR-T MB-101的1期临床数据。所有患者的中位总生存期（中位OS）为8个月，高于复发胶质母细胞瘤的6个月预期生存期，而五项细分亚组中，最大剂量第五组OS达10.2个月。(只剩6个月时，他敲响了CAR-T的门)2.(doi:10.1038/s41591-024-02893-z)同期，吉利德/宾法大学也公布了其靶向EGFR和IL13Rα2的鞘内二价CAR T细胞同样治疗复发性胶质母细胞瘤的1期试验中期结果。该CAR-T会在过表达(EGFR扩增)和/或EGFR胞外结构域突变而发生EGFR 激活失调时激活。接受治疗的6名患者的中位随访时间为2.5个月，所有 6 名患者都观察到增强肿瘤大小的减小，部分肿瘤消退维持在第 +28 天及以后。然而，没有一个达到RANO标准的客观反应标准。IL-15(doi:10.3389/fimmu.2023.1165404)2023年信达生物与苏州大学合作设计的一款H9 CAR-IL15 T细胞，旨在解决其Claudin18.2 CAR-T疗法的长期持久性问题。小鼠 IL-15 修饰赋予肿瘤脉管系统靶向 CAR-T 细胞增强的效应器功能、植入和重编程TME能力，从而在免疫功能正常的小鼠中更好地控制肿瘤。其结果显示，在体外实验中，mIL-15对CAR-T细胞杀伤活性的明显影响仅保持较短时间，在超过三轮刺激后消失，可能和IL-15浓度不足相关。尽管IL-15在体外试验中对CAR-T细胞的总体影响通常不显著，但其在体内的作用是显著的。即使没有淋巴细胞清除预处理，H9 CAR-IL15 T细胞仍然产生明显的肿瘤抑制作用，而未修饰的CAR-T细胞几乎没有效果，从而有机会降低淋巴细胞清除方案造成的毒性作用。IL-24(doi:10.1038/s41416-024-02601-1)IL-24主要是通过靶向肿瘤细胞的干性来影响免疫细胞的疗效，可以通过多种途径特异性诱导肿瘤细胞凋亡，包括DNA损伤、SARI诱导、AIF、ATM和beclin-1轴。同时IL-24还可促进T细胞活化、中枢记忆分化和增殖，但不影响其凋亡。郑州大学研究人员通过构建IL-24 CAR-T细胞，旨在通过IL-24影响抗原丢失的肿瘤细胞对CAR-T的耐药性。体内模型显示，分泌IL-24的CAR-T细胞能更好地控制晚期肿瘤和复发。小结目前很多细胞因子CAR-T都能在安全性方面有较好表现，但实际有效性还是参差不齐的。其他参考文献：Shi H,et al.(2023) IL-15 armoring enhances the antitumor efficacy of claudin 18.2-targeting CAR-T cells in syngeneic mouse tumor models. “双靶点”CAR-T强势来袭，2天内肿瘤奇迹缩小！CAR-T重击实体瘤

2024年伊始，CAR-T研发领域不断传来好消息，近日世界权威期刊《自然医学（nature medicine）》上，报道了首次使用双靶点CAR-T细胞（即CART-EGFR-IL13Rα2细胞）治疗胶质母细胞瘤（GBM），显示出初步活性和安全性，在治疗短短2天内，MRI就已出现肿瘤缩小表现，这意味着CAR-T在治疗实体瘤的有效性、持久性方面再进一层楼，攻克实体瘤指日可待！ ▲截图源自“nature medicine” 双靶点CAR-T具有强大抗肿瘤活性，治疗2天内肿瘤奇迹般缩小 CAR-T细胞疗法已获FDA批准，用于治疗多种血液系统恶性肿瘤。近年来，研究人员一直在努力研发并提升CAR-T细胞，用于对抗各类实体瘤。本次应用的“双靶点”CAR-T细胞，打破了传统CAR-T只具有单一靶点的局限性，针对脑肿瘤常见的两种相关蛋白，即表皮生长因子受体（ EGFR ，存在于约60%的胶质母细胞瘤中）、白细胞介素13受体α2 （ IL13Rα2 ，存在于约75%的胶质母细胞瘤中），进行研发。在1期试验（NCT05168423）中，纳入6例复发性胶质母细胞瘤患者，采用鞘内注射的方式，对患者施用这款“EGFR”和“IL13Rα2”双靶点CAR-T细胞（即CART-EGFR-IL13Rα2）后， 24~48小时复查MRI显示，所有6例患者的肿瘤体积均缩小（详见下图），而且部分患者缩小状态维持数月。此外，神经毒性也在可控范围内。图1 一例胶质母细胞瘤患者治疗前后MRI变化 ▲图源“Penn Medicine News”，版权归原作者所有，如无意中侵犯了知识产权，请联系我们删除综上，CART-EGFR-IL13Rα2细胞在治疗复发性胶质母细胞瘤方面，具有初步的安全性及生物活性。全球肿瘤医生网小编会持续关注后续研究报道，也期望它能早日上市惠及更多的患者。其实除了这款新型双靶点CAR-T细胞外，还有多款CAR-T细胞在实体瘤治疗领域，获得了不俗的疗效，下面简单盘点一下，以供参考。国研GPC3 CAR-T细胞，晚期肝细胞癌患者无瘤生存超7年 ▲截图源自“WILEY Online Library” 国际知名期刊《Cancer Communications（癌症通讯）》，介绍了1例晚期肝细胞癌患者，在接受国研创新——CAR-GPC3 T细胞治疗后，完全缓解长达7年的报道，引起了巨大轰动！这位54岁的男性肝细胞癌患者，既往接受过手术治疗，但效果不佳，术后3个月后癌症复发，在多次治疗后，出现下腔静脉癌栓、腹膜后淋巴结转移等表现。此时对他而言，临床已无更好的治疗方案。抱着最后的希望，他参加了中国进行的CAR-GPC3 T细胞临床试验。这款CAR-T细胞针对的是肝细胞癌中名为“GPC3”的癌胚抗原， GPC3因其在70%~80%的肝细胞癌中高表达，而在正常组织中几乎不表达，因而被认为是精准治疗肝癌细胞的“黄金”靶标！该患者在入组接受CAR-T细胞联合局部治疗2周后，达到了完全缓解（详见下图）。幸运的是，截至目前他已实现7年以上无瘤生存。值得一提的是，在此期间，该患者除口服抗乙型肝炎病毒（HBV）药物治疗外，并未接受其他抗癌治疗。 CAR-T 疗法招募进行时：目前有多款CAR-T临床实验正在进行中，主要针对以下癌种： Claudin18.2 ：用于治疗胃癌，胰腺癌等；间皮素（mesothelin）：用于治疗间皮瘤、胰腺癌、卵巢癌、肺癌； CEA ：用于治疗肺癌、结肠癌、胃癌、乳腺癌和胰腺癌； MUC-1 ：用于治疗肝癌、肺癌、胰腺癌、结肠癌、胃癌; GPC3：用于治疗肝癌; GUCY2C ：用于治疗结直肠癌； PSMA ：用于治疗前列腺癌等； EGFＲvII ：用于治疗神经胶质瘤、头颈部肿瘤； B7-H3 ：用于治疗尤文肉瘤、横纹肌肉瘤、肾母细胞瘤、神经母细胞瘤、髓母细胞瘤、特别难以治疗的脑干肿瘤（DIPG）。上述癌种患者如果想检测相关靶点或寻求CAR-T疗法帮助，可提交资料至全球肿瘤医生网医学部进行初步评估。国研CT041宣战胰腺癌，肿瘤大幅缩小 ▲截图源自“BMC”网站 2023年11月，《血液学与肿瘤学杂志》报道了一例转移性胰腺癌患者，在接受国研CT041治疗后，靶病灶和转移灶明显缩小的的案例。该患者为58岁女性，既往接受标准治疗失败，肿瘤持续进展，已发生全身转移。万念俱灰下，她入组接受了CT041细胞回输治疗。这是一款针对 CLDN18.2（即Claudin18.2，在胰腺癌中高表达）的新型CAR-T细胞产品，既往在治疗消化系统恶性肿瘤中获得了良好的缓解率。该患者在CT041治疗后，幸运的达到部分缓解（PR），靶病灶显著缩小30%以上，同时肺转移也出现了大幅缩小（详见下图）！图患者治疗前后肺部影像学评估 ▲图源“BMC”，版权归原作者所有，如无意中侵犯了知识产权，请联系我们删除注;“★”表示“原发病变”。 CAR-T在其他实体瘤治疗领域大放异彩 PART1 晚期铂耐药卵巢癌，PRGN-3005治疗后疾病控制率高达85.7% PRGN-3005是一种Ultra CAR-T细胞疗法，用于治疗晚期复发性卵巢癌、输卵管癌、原发性腹膜癌等。《临床肿瘤学杂志》发表了“PRGN-3005治疗复发或难治性卵巢癌”的1/1b期临床研究结果，在入组的25例可评估患者中，其中位年龄为64岁(38-76)，既往接受过8种治疗方案的大量预处理。将这些患者分为三组，即环磷酰胺LD治疗组（IVLD，n=7）、经腹膜内(IP)输注组（C1，n=12）、经静脉(IV)输注组（C2，n=6）。结果显示，IVLD组患者首次再分期时的疾病控制率(DCR)高达85.7%，CA125平均降低了27.4% ，靶肿瘤负荷降低57% (4/7)。在接受第二次PRGN-3005回输治疗后，1例患者的目标肿瘤负荷减少28% 。且未发生与PRGN-3005相关的严重不良反应，如≥3级细胞因子释放综合征(CRS)、神经毒性、剂量限制性毒性等。综上，PRGN-3005 Ultra CAR-T细胞疗法，在治疗卵巢癌中获得了较为理想的疾病控制率（DCR）率，且耐受性良好、总体肿瘤负荷减少。 PART2 结直肠癌：GCC19CART治疗后，客观缓解率达50% GCC19CART是由上海斯丹赛生物公司研发的一款CAR-T细胞产品，已获得美国食品和药物管理局（FDA）授予的快速通道资格，用于治疗结直肠癌。根据实体肿瘤疗效评价标准，13例1级剂量组患者的客观缓解率（ORR）达15.4% (2/13)，8例2级剂量组患者的 ORR达50% (4/8)！小编寄语美国生物学家乔治•戴利曾说过：“如果说20世纪是药物治疗的时代，那么21世纪就是细胞治疗的时代”。随着医学技术的不断发展，以过继性免疫细胞治疗、癌症疫苗等为代表的肿瘤免疫疗法，也成了继手术、放疗、化疗后的第四种肿瘤治疗方法。其中就不得不提到获得突破性进展的CAR-T细胞疗法，随着多款产品相继获批上市，CAR-T进入井喷式的发展模式。目前国内共有24家企业布局CAR-T疗法，包括驯鹿医疗、传奇生物、复星凯特科济生物、药明巨诺、信达生物、西比曼生物等。研究人员也在不断探索新的靶点、甚至开发双靶点，以尝试用于多款实体瘤的治疗。全球肿瘤医生网小编也希望在各国专家的共同努力下，CAR-T细胞可以早日突破价格和实体瘤的瓶颈，让更多晚期癌症患者受益！参考资料 [1]Bagley SJ,et al.Intrathecal bivalent CAR T cells targeting EGFR and IL13Rα2 in recurrent glioblastoma: phase 1 trial interim results.
