Senses fluctuations in the circulating calcium concentration and modulates the production of parathyroid hormone (PTH) in parathyroid glands (By similarity). The activity of this receptor is mediated by a G-protein that activates a phosphatidylinositol-calcium second messenger system (PubMed:7759551). The G-protein-coupled receptor activity is activated by a co-agonist mechanism: aromatic amino acids, such as Trp or Phe, act concertedly with divalent cations, such as calcium or magnesium, to achieve full receptor activation (PubMed:27434672, PubMed:27386547).

Phase 2 Study of the PTH-Independent Effects of Encaleret on Mineral Homeostasis in Subjects With Post-Surgical Hypoparathyroidism (PSH)

Background:
Parathyroid glands in the neck make a hormone that keeps blood calcium levels stable. Sometimes these glands are damaged or removed during neck surgery. This can lead to a condition called postsurgical hypoparathyroidism (PSH). People with PSH have low levels of calcium in their blood. Calcium and vitamin D pills can help them keep their blood calcium levels steady. But this can increase calcium in the urine and result in kidney problems. New treatments for PSH are needed.
Objective:
To test a drug (encaleret) in people with PSH.
Eligibility:
People aged 18 or older who have PSH.
Design:
Participants will be in the study for 6 months. They will have a screening visit and a treatment visit.
Screening will take up to 2 days. Participants will have a physical exam. They will have blood and urine tests and tests of their heart function. They will have an ultrasound of their kidneys; they will lie on a table for 15 to 30 minutes while a wand is moved over their back.
Treatment will require participants to stay in the clinic for 7 days and 6 nights. They will take the study drug (encaleret) by mouth twice a day for 5 days. They will have a small, flexible tube inserted into a vein; this will remain in place during the visit. Blood samples will be taken through the tube 4 to 9 times each day. Participants urine will be collected.
Participants will have follow-up blood tests 1 week after leaving the clinic. They will have 3 follow-up phone calls.

Parathyroid Hormone Level and Growth in Pediatric Patients With ESRD on Regular Hemodialysis

Chronic kidney disease (CKD) is defined as either renal damage and/ or a glomerular filtration rate (GFR) below 60 mL/min/1.73 m2 for more than 3 months. End stage renal disease (ESRD) is defined as GFR below 15 mL/min/1.73 m2.
Secondary hyperparathyroidism (SHPT) is an insidious disease that develops early in the course of CKD and increase in severity as the GFR deteriorates. High serum levels of intact parathyroid hormone (iPTH) are known to cause high turnover bone disease, i.e. osteitis fibrosa and have also been reported to increase the mortality risk in patients undergoing hemodialysis (HD).
Standard therapy for SHPT includes dietary calcium supplementation, active vitamin D, and phosphate binders; however, these are often insufficient to allow patients to achieve their serum parathyroid hormone (PTH), calcium and calcium-phosphorus product (Ca × P) targets.
Recent preclinical studies have demonstrated that treatment with calcimimetics that increase the sensitivity of the calcium-sensing receptor (CaR) to calcium can reverse the alterations in CaR and vitamin D receptor expression and parathyroid cell proliferation that are associated with SHPT.
Calcimimetics such as cinacalect are positive allosteric modulators of the calcium-sensing receptor that increase its sensitivity by lowering the threshold for activation by extracellular calcium ions. The calcimimetic cinacalcet mainly increases the sensitivity of the CaR to extracellular Ca, thus inhibiting the release of PTH, although, as recently shown, it also decreases PTH synthesis.
Growth hormone (GH) indirectly promotes the growth of child by stimulating the production of insulin like growth factor (IGF-1), many children with renal disease have normal or elevated level of GH in their blood otherwise the levels of IGF-1 are low because the approximately 98% of IGF-1 is always bound to one of binding proteins (BP) inside the liver, so this accumulated protein will reduce the function of IGF-1.
Using cinacalcet in controlling hyperparathyroidism may reduce growth problems in children with ESRD with regular hemodialysis.

A Phase 3 Randomized, Double-Blind, Placebo-Controlled Study Evaluating the Efficacy and Safety of Dose-Titrated PLS240 in the Treatment of Secondary Hyperparathyroidism in Individuals With End Stage Kidney Disease on Hemodialysis (PATH-2) With an Open-Label Extension

This study is to evaluate the efficacy and safety of PLS240 in patients with hemodialysis-dependent end stage kidney disease (ESKD) and secondary hyperparathyroidism (SHPT). The study consists of two phases. First, a placebo-controlled, double-blind phase where patients will be randomly assigned to either receive dose-titrated PLS240 or matching placebo for 27 weeks. After the completion of the double-blind phase, patients will be eligible to enroll in the open-label extension phase, where they will receive dose-titrated PLS240 for an additional 26 weeks. Throughout the duration of the study, patients will be expected to attend multiple study visits where an investigator will collect blood, preform electrocardiograms (ECGs) and physical exams, and further assess the safety and efficacy of PLS240.

A novel mouse model for familial hypocalciuric hypercalcemia (FHH1) reveals PTH-dependent and independent CaSR defects.

The Calcium-sensing receptor (CaSR) senses extracellular calcium, regulates parathyroid hormone (PTH) secretion, and has additional functions in various organs related to systemic and local calcium and mineral homeostasis. Familial hypocalciuric hypercalcemia type I (FHH1) is caused by heterozygous loss-of-function mutations in the CaSR gene, and is characterized by the combination of hypercalcemia, hypocalciuria, normal to elevated PTH, and facultatively hypermagnesemia and mild bone mineralization defects. To date, only heterozygous Casr null mice have been available as model for FHH1. Here we present a novel mouse FHH1 model identified in a large ENU-screen that carries an c.2579 T > A (p.Ile859Asn) variant in the Casr gene (Casr^BCH002 mice). In order to dissect direct effects of the genetic variant from PTH-dependent effects, we crossed Casr^BCH002 mice with PTH deficient mice. Heterozygous Casr^BCH002 mice were fertile, had normal growth and body weight, were hypercalcemic and hypermagnesemic with inappropriately normal PTH levels and urinary calcium excretion replicating some features of FHH1. Hypercalcemia and hypermagnesemia were independent from PTH and correlated with higher expression of claudin 16 and 19 in kidneys. Likewise, reduced expression of the renal TRPM6 channel in Casr^BCH002 mice was not dependent on PTH. In bone, mutations in Casr rescued the bone phenotype observed in Pth null mice by increasing osteoclast numbers and improving the columnar pattern of chondrocytes in the growth zone. In summary, Casr^BCH002 mice represent a new model to study FHH1 and our results indicate that only a part of the phenotype is driven by PTH.

Single-cell spatial transcriptomic and translatomic profiling of dopaminergic neurons in health, aging, and disease.

The brain is spatially organized and contains unique cell types, each performing diverse functions and exhibiting differential susceptibility to neurodegeneration. This is exemplified in Parkinson's disease with the preferential loss of dopaminergic neurons of the substantia nigra pars compacta. Using a Parkinson's transgenic model, we conducted a single-cell spatial transcriptomic and dopaminergic neuron translatomic analysis of young and old mouse brains. Through the high resolving capacity of single-cell spatial transcriptomics, we provide a deep characterization of the expression features of dopaminergic neurons and 27 other cell types within their spatial context, identifying markers of healthy and aging cells, spanning Parkinson's relevant pathways. We integrate gene enrichment and genome-wide association study data to prioritize putative causative genes for disease investigation, identifying CASR as a regulator of dopaminergic calcium handling. These datasets represent the largest public resource for the investigation of spatial gene expression in brain cells in health, aging, and disease.

Janus Membrane with Intrafibrillarly Strontium-Apatite-Mineralized Collagen for Guided Bone Regeneration.

Commercial collagen membranes face difficulty in guided bone regeneration (GBR) due to the absence of hierarchical structural design, effective interface management, and diverse bioactivity. Herein, a Janus membrane called SrJM is developed that consists of a porous collagen face to enhance osteogenic function and a dense face to maintain barrier function. Specifically, biomimetic intrafibrillar mineralization of collagen with strontium apatite is realized by liquid precursors of amorphous strontium phosphate. Polycaprolactone methacryloyl is further integrated on one side of the collagen as a dense face, which endows SrJM with mechanical support and a prolonged lifespan. In vitro experiments demonstrate that the dense face of SrJM acts as a strong barrier against fibroblasts, while the porous face significantly promotes cell adhesion and osteogenic differentiation through activation of calcium-sensitive receptor/integrin/Wnt signaling pathways. Meanwhile, SrJM effectively enhances osteogenesis and angiogenesis by recruiting stem cells and modulating osteoimmune response, thus creating an ideal microenvironment for bone regeneration. In vivo studies verify that the bone defect region guided by SrJM is completely repaired by newly formed vascularized bone. Overall, the outstanding performance of SrJM supports its ongoing development as a multifunctional GBR membrane, and this study provides a versatile strategy of fabricating collagen-based biomaterials for hard tissue regeneration.

试验组与健康代谢相关细菌比例增加，试验期间没有出现相关并发症。【主编评语】代谢性疾病是由于代谢出现问题产生的疾病，可以是代谢障碍也可以是代谢增强，主要是指糖、蛋白质、脂肪、维生素等营养物质的代谢。近日，意大利研究人员在Gut发表最新研究，开展一项动物实验，使用球囊联合十二指肠支架，可能比胃旁路手术更有效实现逆转糖尿病以及减少患者体重，而且它无需外科手术，也无需对内脏器官进行任何切割即可实现这一目标，值得关注。（@九卿臣）【原文信息】 研究揭示钙敏感受体CaSR非对称激活的分子机制

综合前期研究成果，研究人员提出了完整的CaSR非对称激活机制，完善了人们对C家族GPCR激活机制的理解，同时为靶向CaSR的精准调控药物研发提供了重要理论基础。近日，中国科学技术大学生命科学与医学部教授田长麟课题组首次解析了人源钙敏感受体（Ca2+Sensing Receptor，CaSR）与下游信号传导蛋白Gq的高分辨三维复合物结构，结合细胞信号转导和核磁共振（NMR）揭示了CaSR蛋白受激动剂、正向别构调节剂等分子非对称激活的分子机制，相关研究成果以Structural insights into asymmetric activation of the calcium-sensing receptor-Gq complex为题在线发表在《细胞研究》（Cell Research）上。钙离子敏感受体（CaSR）是C家族G蛋白偶联受体（GPCR）的一员，广泛分布于甲状旁腺、肠道、骨骼和肾脏等组织器官中。CaSR感应血液中钙离子的浓度，并通过平衡钙离子的吸收和外排从而维持人体中的钙离子平衡。由于钙离子参与信号传递、凝血、肌肉收缩、骨骼生成等众多重要生理活动，CaSR维持血钙稳定对人体生命活动至关重要，其功能异常将会导致各种疾病。 CaSR是人体中非常重要的药物靶标蛋白，目前已经有多个正向变构调节剂药物成功获批上市，它们分别结合在CaSR受体不同的靶点上：小分子药物cinacalcet和evocalcet结合在跨膜结构域，多肽药物Etelcalcetide结合在胞外VFT结构域，小分子药物Upacicalcet则结合在胞外的芳香族氨基酸结合口袋中。CaSR激活的分子机制研究将进一步助力靶向该受体的药物研发。针对CaSR这一重要的药物靶标蛋白，该团队开展了长期系统性研究，2021年在国际上首次解析了全长CaSR蛋白的三维结构，揭示了受体在钙离子和芳香族氨基酸两种激动剂协同作用下，从非活性构象转变成活性构象的结构机制。在此之后，国际上多个团队分别发表了CaSR受体在无配体、多个配体或别构调节剂结合的三维结构，分别提出了对称激活假说以及非对称激活假说。然而，由于缺少CaSR与下游信号蛋白G蛋白三聚体的复合物高分辨结构，其完整的激活机制仍然缺失。经过近2年多努力，田长麟团队再次获得了CaSR-Gq复合物的高分辨冷冻电镜三维结构，结合细胞信号转导和NMR等实验揭示了CaSR在激活状态下结合Gq并启动下游信号传导的非对称激活机制。CaSR-Gq复合物结构也是目前第一个C家族GPCR与Gq结合的三维结构，揭示了其独特特点：（1）有别于A家族或B家族GPCR的Gq蛋白结合模式，其中Gq蛋白的α5螺旋没有插入CaSR胞内侧的跨膜螺旋深处，而是结合在受体胞内侧的一个非常浅的口袋中（主要由受体胞内loop和TM3胞内末端组成）；（2）CaSR-Gq的结合模式与mGLuR-Gi, GABAB-Gi复合物的差异，体现了Gq与受体的特异性结合界面。该团队结合冷冻电镜与NMR技术，首次揭示了正向别构调节药物分子cinacalcet在受体信号复合物中的多样结合模式：cinacalcet以伸展（extended）和弯曲（bent）两种不同的构象，分别结合在二聚体CaSR受体的两个跨膜结构域的胞外侧结合口袋中，其中只有结弯曲-cinacalcet的跨膜结构域的胞内能够偶联下游信号蛋白Gq。和其他GPCR（包括A家族、B家族、C家族等）类似，CaSR仅结合一个下游信号蛋白，且CaSR-Gq复合物整体处于非对称构象，而且受体的局部跨膜结构域也处于非对称构象，cinacalcet以两个完全不同的构象结合在两个跨膜结构域中。同时，CaSR单独受体（不含Gq蛋白）与cinacalcet的三维结构表明，尽管cinacalcet在两个跨膜结构域中都处于bent构象，但是CaSR受体本身仍然是处于非对称构象。综合前期研究成果，研究人员提出了完整的CaSR非对称激活机制，完善了人们对C家族GPCR激活机制的理解，同时为靶向CaSR的精准调控药物研发提供了重要理论基础。相关研究工作得到国家自然科学基金、科学技术部、中国科学院和安徽省自然科学基金的资助。图1 CaSR在激动剂、别构调节剂作用下与下游信号转导蛋白Gq的非对称结合模式
