[Clinical and prognostic characteristics of pediatric acute myeloid leukemia with myelodysplasia-related changes under different diagnostic criteria].

Objective: To evaluate the clinical and prognostic differences in acute myeloid leukemia with myelodysplasia-related changes (AML-MRC) children under different diagnostic criteria (World Health Organization (WHO) 2016 and WHO 2022 criteria). Methods: In this retrospective cohort study, clinical characteristics and prognosis information of 260 acute myeloid leukemia (AML) children admitted to Institute of Hematology & Blood Diseases Hospital, Chinese Academy of Medical Sciences from August 2017 to August 2021 were analyzed retrospectively. According to WHO 2016 and WHO 2022 diagnostic criteria, patients were divided into AML-MRC group and non-AML-MRC group, the prognostic and genetic differences between two groups were compared respectively. Meanwhile, the characteristics of children with 8 MRC-related genes defined in WHO 2022 diagnostic criteria were described. Mann-Whitney U test, chi-square test were used for comparison between groups. Survival curve was plotted by Kaplan-Meier method, and comparison between groups was performed by Log-Rank method. Results: Among the 260 children, there were 148 males and 112 females. The follow-up time was 26 (16, 38) months. A total of 28 children (10.8%) were diagnosed with AML-MRC according to the WHO 2016 diagnostic criteria. Compared with non-AML-MRC children, the frequency of PTPN11, RUNX11, SH2B3, MPL and STAG2 mutations was higher in AML-MRC children (25.0% (7/28) vs.4.3% (10/232), 14.3% (4/28) vs.3.9% (9/232), 10.7% (3/28) vs.2.2% (5/232), 10.7% (3/28) vs.2.2% (5/232), 10.7% (3/28) vs.0.9% (2/232), all P<0.05). The 2-year overall survival (OS) and events free survival (EFS) rate of 28 AML-MRC children under WHO 2016 diagnostic criteria were worse than those of 232 non-AML-MRC children ((62.1±10.8)% vs. (94.5±1.6)%, χ²=22.1,P<0.001;(48.0±10.6)% vs. (70.9±3.2)%, χ²=6.33,P=0.012). Twenty-seven children (10.4%) were eventually diagnosed with AML-MRC according to WHO 2022 criteria, their 2-year OS rate were worse than 233 non-AML-MRC children ((60.8±11.1)% vs. (94.5±1.6)%, χ²=24.49,P<0.001), and there was no statistically significant difference in EFS rate between two groups at 2 years ((55.1±10.8)% vs. (70.1±3.2)%, χ²=2.44, P=0.119). Conclusions: Compared with the 2022 WHO diagnostic criteria, the survival rates of children with AML-MRC under the 2016 WHO diagnostic criteria were worse than that of children without MRC.The new version of the AML-MRC diagnostic criteria emphasizes the importance of genes.

Over the past decade, US Food and Drug Administration (FDA)-approved immune checkpoint inhibitors that target programmed death-1 (PD-1) have demonstrated significant clinical benefit particularly in patients with PD-L1 expressing tumors. Toripalimab is a humanized anti-PD-1 antibody, approved by FDA for first-line treatment of nasopharyngeal carcinoma in combination with chemotherapy. In a post hoc analysis of phase 3 studies, toripalimab in combination with chemotherapy improved overall survival irrespective of PD-L1 status in nasopharyngeal carcinoma (JUPITER-02), advanced non-small cell lung cancer (CHOICE-01) and advanced esophageal squamous cell carcinoma (JUPITER-06). On further characterization, we determined that toripalimab is molecularly and functionally differentiated from pembrolizumab, an anti-PD-1 mAb approved previously for treating a wide spectrum of tumors. Toripalimab, which binds the FG loop of PD-1, has 12-fold higher binding affinity to PD-1 than pembrolizumab and promotes significantly more Th1- and myeloid-derived inflammatory cytokine responses in healthy human PBMCs in vitro. In an ex vivo system employing dissociated tumor cells from treatment naïve non-small cell lung cancer patients, toripalimab induced several unique genes in IFN-γ and immune cell pathways, showed different kinetics of activation and significantly enhanced IFN-γ signature. Additionally, binding of toripalimab to PD-1 induced lower levels of SHP1 and SHP2 recruitment, the negative regulators of T cell activation, in Jurkat T cells ectopically expressing PD-1. Taken together, these data demonstrate that toripalimab is a potent anti-PD-1 antibody with high affinity PD-1 binding, strong functional attributes and demonstrated clinical activity that encourage its continued clinical investigation in several types of cancer.

Macrophage SHP2 Deficiency Alleviates Diabetic Nephropathy via Suppression of MAPK/NF-ĸB-Dependent Inflammation.

Increasing evidence implicates chronic inflammation as the main pathological cause of diabetic nephropathy (DN). Exploration of key targets in the inflammatory pathway may provide new treatment options for DN. Here, we aim to investigate the role of Src Homology 2 Containing Protein Tyrosine Phosphatase 2 (SHP2) in macrophages and its association with DN. The upregulated phosphorylation of SHP2 was detected in macrophages of both diabetic patients and mouse model. Using the macrophage-specific SHP2 knockout mice (SHP2-MKO) and SHP2fl/fl mice injected with streptozotocin (STZ), we showed that SHP2-MKO significantly attenuated renal dysfunction, collagen deposition, fibrosis, and inflammatory response in STZ-induced diabetic mice. RNA-sequencing analysis using primary mouse peritoneal macrophages (MPMs) showed that SHP2 deletion mainly affects mitogen-activated protein kinases (MAPKs) and nuclear factor kappa B (NF-ĸB) signaling pathway and MAPKs/NF-ĸB-dependent inflammatory cytokine release in MPMs. Further study indicated that SHP2-deficient macrophages failed to release cytokines that induce phenotype transition and fibrosis in renal cells. Administration with a pharmacological SHP2 inhibitor, SHP099, remarkably protected kidneys in both type 1 and type 2 diabetic mice. In conclusion, these results identify macrophage SHP2 as a new accelerator of DN and suggest that SHP2 inhibition may be a therapeutic option for DN patients.

诺诚健华：2023年研发费用增长17.5%，今年有望递交4个NDA！

▎药明康德内容团队报道3月28日，诺诚健华公布了2023年业绩报告和公司进展。2023年，该公司研发费用同比增加17.5%，达到7.5亿元，研发费用的增加主要是持续推进重要管线的全球临床试验和加大早期研发产品投资。此外，根据诺诚健华新闻稿，该公司在2024年有望递交4个新药NDA。血液瘤管线产品进展奥布替尼（BTK抑制剂）奥布替尼2023年4月获批用于既往至少接受过一次治疗的边缘区淋巴瘤（MZL）患者，成为奥布替尼获批的第三个适应症。奥布替尼一线治疗慢性淋巴细胞白血病（CLL）/小淋巴细胞淋巴瘤（SLL）的3期注册临床试验2023年上半年完成患者入组，预计2024年第三季度递交新药上市申请（NDA）。在美国，奥布替尼用于既往至少接受过一次治疗的套细胞淋巴瘤（MCL）患者的 2期注册临床试验2023年上半年完成患者入组，预计2024年第三季度向美国FDA递交NDA上市申请。诺诚健华正启动奥布替尼一线治疗MCL的全球3期临床试验。奥布替尼在新加坡获批用于既往至少接受过一次治疗的MCL患者，标志着诺诚健华商业化走向国际。预计2024年将在新加坡提交既往至少接受过一次治疗的MZL的NDA上市申请。诺诚健华正启动奥布替尼针对MZL的三期确证性临床试验。联合BCL2抑制剂ICP-248一线治疗CLL/SLL。奥布替尼一线治疗弥漫大B细胞淋巴瘤（DLBCL）MCD亚型3期注册研究目前在中国45个中心开展。坦昔妥单抗（tafasitamab，CD19单抗）坦昔妥单抗联合来那度胺治疗复发/难治性DLBCL注册临床试验在中国完成患者入组，预计2024年第二季度递交BLA上市申请，预计2025年上半年获批上市。坦昔妥单抗治疗方案在中国香港地区获批上市，并在海南博鳌和粤港澳大湾区获批使用，并分别在两地先行区的医院开出首方。ICP-248（BCL2抑制剂）ICP-248的初步研究结果展示了良好的安全性和药代动力学（PK），展示了和其他BCL2抑制剂的差异化。截至目前，17名患者完成给药，在完成评估的六名患者中，总缓解率（ORR）达到100%，三名患者达到完全缓解（CR），其中两名患者的微小残留病灶已检测不到（uMRD）。2024年1月，ICP-248获美国FDA批准开展临床研究。2024年3月，ICP-248联合奥布替尼一线治疗CLL/SLL在中国获批临床。ICP-248是一款新型口服高选择性BCL2抑制剂，旨在单药或与BTK抑制剂等其他药物联合治疗CLL/SLL、MCL、急性髓性白血病（AML）和其他NHL患者。ICP-B02（CD20xCD3双特异性抗体）ICP-B02（CM355）是诺诚健华和康诺亚合作开发的CD20xCD3双特异性抗体。初步数据显示，ICP-B02的静脉输注（IV）制剂和皮下注射（SC）制剂在滤泡性淋巴瘤（FL）和DLBCL患者中都展示了良好的疗效。在接受至少一剂6毫克以上剂量的13名患者中，ORR为100%。9名可评估的皮下注射患者中，7名患者完全缓解（CR），其中2名DLBCL患者完全缓解。基于ICP-B02单药令人鼓舞的结果，诺诚健华计划在更早线NHL患者中开展ICP-B02联合其他免疫化疗的剂量扩展研究。目前，诺诚健华已提交联合疗法的新药研究申请（IND）。ICP-490（CRBN E3连接酶调节剂）ICP-490是诺诚健华自主研发的创新药，源自分子胶平台，旨在治疗MM等血液肿瘤。诺诚健华正在进行多发性骨髓瘤（MM）患者1期剂量递增研究，展示了良好的安全性和耐受性。药效学（PD）分析显示主要药理靶点Aiolos（IKZF3）和Ikaro（IKZF1）的降解更深。2023年9月，ICP-490联合地塞米松在中国获批临床。作为单一疗法或与其他疗法联用，ICP-490有望在血液肿瘤领域展现强劲的潜力。ICP-B05（CCR8单抗）ICP-B05（CM369）是一款抗趋化因子受体8（CCR8）单克隆抗体，由诺诚健华和康诺亚共同开发，拟作为单一疗法或与其他疗法联合治疗高发的晚期实体瘤和NHL。在正在进行的1期临床试验中，该药展示了良好的耐受性，未观察到剂量限制毒性（DLT）。初步结果展现了良好的PK，同时观察到作为药效学（PD）生物标志物的Treg被清除。实体瘤研究中的剂量递增已升至150毫克，这也是针对NHL患者的初始剂量。ICP-B05在NHL患者中观察到初步疗效。自身免疫性疾病管线进展自身免疫性疾病几乎可以影响身体的每个器官，且在生命的任何阶段都会发生。诺诚健华正进一步加强药物发现平台，通过B细胞和T细胞通路开发自身免疫性疾病领域的全球前沿靶点，打造差异化的自身免疫性疾病管线，旨在为大量未满足的临床需求提供创新疗法。奥布替尼奥布替尼治疗原发免疫性血小板减少症（ITP）已获得概念验证（PoC），并启动3期注册临床，预计2024年年底完成患者入组。2023年6月，奥布替尼治疗ITP的2期研究成果在第28届欧洲血液学协会（EHA）年会上进行口头报告。最新数据显示，50毫克组别中，40%的患者达到主要终点。在达到主要终点的患者中，83.3%的患者获得持久缓解。在对糖皮质激素（GC）或静脉注射免疫球蛋白（IVIG）敏感的患者中，50毫克剂量组中，75.0%的患者达到主要终点。奥布替尼治疗系统性红斑狼疮（SLE）的2a期临床试验取得积极效果，研究显示SLE反应指数-4（SRI-4）呈剂量依赖性的改善趋势，同时观察到尿蛋白呈下降趋势。2b期试验持续推进中，预计2024年年中完成患者入组。奥布替尼治疗多发性硬化（MS）全球2期试验的24周数据与之前报告的12周的数据在有效性和安全性方面趋势均一致。奥布替尼治疗MS三个剂量组的24周数据都达到主要终点，奥布替尼显著降低了多发性硬化（MS）患者的疾病活动，呈现剂量依赖性（Cmax driven）的改善趋势。与安慰剂组（第12周后每天一次50毫克奥布替尼）相比，每天一次80毫克剂量组在第24周时钆增强T1脑部累计新发病灶（Gd+ T1）减少了92.3%。与其他已批准或处于开发阶段的MS药物相比，这种减少最为显著。所有奥布替尼剂量组在治疗4周后均达到T1新病灶控制，且效果可持续长达24周。每天一次80毫克剂量组显示了最优的有效性，并且在整个24周内病灶控制效果最佳，安全性也最好，这表明每天一次80毫克有潜力成为治疗MS的最佳剂量。奥布替尼治疗视神经脊髓炎谱系疾病（NMOSD）的2期临床试验正在中国进行。ICP-332ICP-332治疗中重度特应性皮炎（AD）2期临床的最新数据在2024年美国皮肤病学会（AAD）年会以重磅口头报告形式发布。ICP-332展示了较好的疗效和安全性，达到多个有效性终点，包括EASI（湿疹面积和严重程度指数）较基线变化百分比、EASI 50、EASI 75、EASI 90（EASI评分较基线改善≥50%，75%，90%）、研究者整体评估（IGA）0/1（即皮损完全清除或基本清除）且较基线改善≥2分、以及瘙痒NRS评分较基线变化等。80毫克和120毫克两个治疗组EASI评分较基线改善的百分比变化分别为78.2%和 72.5%，与安慰剂组的16.7%相比，具备显著的统计学差异；两个治疗组EASI 75应答率均为64.0%，较安慰剂组（8.0%）改善了56.0%，优于多种获批创新药物单药治疗12周或16周的疗效（非头对头比较）。ICP-332 80毫克组EASI 90和IGA 0/1且较基线改善≥2分的应答率较安慰剂组分别改善40.0%和32.0%，具有统计学意义。ICP-332展示了良好的安全性和耐受性。两个治疗组的总体不良事件发生率、感染及侵染类疾病的不良事件发生率均与安慰剂组相当。预计2024年在中国启动ICP-332治疗AD的3期临床试验，在美国启动临床试验，以及针对第二个适应症白癜风的2期试验。ICP-332是一种新型酪氨酸激酶2（TYK2）抑制剂，开发用于治疗各种T细胞相关的自身免疫性疾病。ICP-488已完成ICP-488治疗银屑病患者的1期研究，在接受治疗4周的患者中，ICP-488展示了有效性。在每日一次6毫克剂量组中，银屑病面积和严重程度指数（PASI）评分较基线变化百分比的最小二乘均值改善38%，而安慰剂组为14%，具有统计学意义。6毫克组PASI 50的应答率为42%。已完成单剂量爬坡（1毫克到36毫克）和多剂量爬坡（2毫克到12毫克）研究，观察到ICP-488血浆暴露与剂量成比例。给药期同时服用标准高脂肪、高热量膳食，没有观察到明显的食物效应。ICP-488展示了良好的安全性和耐受性，治疗组与安慰剂组的不良事件相当。ICP-488治疗银屑病的2期研究预计2024年上半年完成患者入组。ICP-488是一款强效的高选择性TYK2变构抑制剂，通过结合TYK2 JH2结构域，阻断IL-23、IL-12和I型干扰素等炎性细胞因子的信号转导。ICP-B02（CM355）在1/2期临床试验中，ICP-B02在复发难治性NHL患者中首次输注（SC和IV）后，诱导外周B细胞的深度且持续清除。鉴于B细胞在多种严重自身免疫性疾病中发挥的关键作用，ICP-B02有潜力在严重的自身免疫性疾病中具有更广泛的应用。ICP-923（新靶点）IL-17是一种促炎细胞因子，在免疫应答中发挥重要作用。靶向IL-17的口服小分子对患者而言更便利，可替代IL-17靶向单克隆抗体。这款新型口服小分子可有效阻断IL-17AA和IL-17AF与IL-17R的结合。创新的实体瘤管线通过靶向治疗和免疫肿瘤学方法，诺诚健华致力于拓展管线的深度和广度，涵盖实体瘤疾病领域。为了使更广泛的患者受益，诺诚健华开发的早期管线包括基石疗法ICP-192、ICP-189和ICP-B05，有望为实体瘤患者提供新的治疗解决方案。Zurletrectinib（ICP-723）诺诚健华正加速推进zurletrectinib在中国的注册临床试验，预计2024年年底递交NDA。截至报告期，zurletrectinib展示了良好的有效性和安全性，ORR为80%~90%。Zurletrectinib被证明可以克服第一代TRK抑制剂的耐药性，为接受第一代TRK抑制剂后耐药的患者带来希望。除成年患者之外，zurletrectinib在青少年（12周岁到18周岁）患者和儿童患者（2周岁到12周岁）中开展临床研究，已覆盖所有年龄的NTRK基因融合阳性患者。儿童患者入组正进行中，已观察到部分缓解。ICP-189诺诚健华与ArriVent达成临床合作协议，ICP-189联合第三代EGFR抑制剂伏美替尼治疗晚期或转移性非小细胞肺癌（NSCLC）患者的疗效和安全性。这一联合用药临床研究正推进中。ICP-189是诺诚健华自主研发的新型蛋白酪氨酸磷酸酶SHP2变构抑制剂，可用作单药疗法及/或和其他抗肿瘤药的联合疗法，旨在为肺癌、头颈癌及消化道肿瘤等实体瘤提供新的临床治疗方法。ICP-189单药治疗观察到初步疗效。截至报告期，剂量已增至120毫克，未观察到DLT，并显示出良好的PK和长半衰期。160毫克剂量组的患者入组正在进行中。SHP2抑制剂是和多种靶向药和免疫疗法联合治疗实体瘤的理想药物。ICP-189在KRAS G12C突变和EGFR过表达驱动的肿瘤模型具有显著的抗肿瘤作用。Gunagratinib（ICP-192）2023年ASCO-GI会议公布gunagratinib治疗胆管癌（CCA）患者的最新临床数据。在既往接受过治疗的局部晚期或转移性的FGFR2基因融合/重排的胆管癌患者中，gunagratinib显示出良好的安全性和有效性。参考资料：[1]诺诚健华2023年业绩报告：以高质量创新药赋能新质生产力发展. Jacobio Pharma Announces 2023 Annual Results

BEIJING, SHANGHAI and BOSTON, March 28, 2024 Jacobio/PRNewswire/ -- Pharma (1167.HK), a clinical-stage oncology company drugging the undruggable targets, today announced its 2023 annual results. The revenue was RMB63.5 million, the R&D investment was RMB372 million, the cash and cash equivalent at the end of 2023 was RMB 1.2 billion. Jacobio Pharma also announced its recent business progress and expected milestones. Dr. Wang Yinxiang, Chairman and CEO of Jacobio Pharma, said: "In the past year, Jacobio continued to make progress in our projects. We received approval for registrational phase III clinical trial of the combination therapy between our SHP2 inhibitor JAB-3312 and KRAS G12C inhibitor glecirasib. Development of core clinical stage products KRAS G12C inhibitor Glecirasib (JAB-21822) Non-small cell lung cancer (NSCLC) The patient enrollment for pivotal trial of glecirasib monotherapy in ≥2L NSCLC patients harboring KRAS G12C mutation was completed. The NDA application is expected to be submitted to CDE (Center for drug evaluation, NMPA) in Q2 2024 as planned. 1L NSCLC is in combination with SHP2 inhibitor JAB-3312. The phase III registrational trial was approved by CDE. Pancreatic cancer (PDAC) The pivotal trial of glecirasib monotherapy in ≥2L PDAC patients was activated in China. The results were presented at the 2024 American Society of Clinical Oncology (ASCO) GI Annual Meeting. The cORR was 41.9% (13/31) and the DCR was 93.5% (29/31). The median progression-free survival (mPFS) was 5.6 months. Colorectal cancer (CRC) The clinical results of glecirasib monotherapy and glecirasib combined with cetuximab in advanced colorectal cancer were presented at the Second JCA- AACR Precision Medicine International Conference. Phase III pivotal trial design of glecirasib monotherapy or glecirasib in combination with cetuximab is expected to be approved by CDE in Q2 2024. Multi-tumors basket Multi-tumors basket includes biliary tract cancer, gastric cancer, small bowel cancer, appendices cancer, etc. The results were presented at the 2024 American Society of Clinical Oncology (ASCO) GI Annual Meeting. The cORR was 57.9%（11/19), DCR was 84.2%（16/19), mPFS was 7.0 months. A phase II single arm pivotal trial is under communication with CDE. SHP2 inhibitor JAB-3312 The Phase III pivotal trial of JAB-3312 in combination with glecirasib to treat 1L NSCLC patients has been approved by CDE, and this study in China is expected to initiated in Q3 2024. JAB-3312 is the very first SHP2 inhibitor entering a phase III registrational trial worldwide. The clinical data of glecirasib in combination with JAB-3312 was published at the 2023 European Society for Medical Oncology Congress (ESMO 2023). Glecirasib (800mg once daily) + JAB-3312 2mg (once daily for 1 week on, then 1 week off) dosage yielded ORR of 86.7% (13/15) and DCR of 100% (15/15). Long term safety and efficacy data have submitted to the 2024 American Society of Clinical Oncology (ASCO) Annual Meeting. Development of other clinical products P53 Y220C activator JAB-30355: The IND has been approved by U.S. FDA (Food and Drug Administration), and phase I clinical trial is expected to be initiated in the second half of 2024. Preclinical data will be presented at the AACR Annual Meeting 2024. BET inhibitor JAB-8263: A Phase II trial of JAB-8263 monotherapy or combination therapies is planned to be initiated in the second half of 2024. Clinical data will be published at the 2024 European Hematology Association (EHA) Congress. Aurora A inhibitor JAB-2485: RP2D is anticipated to be determined in Q2 2024. The preclinical study of JAB-2485 was presented at the 2023 AACR. Anti-CD73 humanized monoclonal antibody JAB-BX102: RP2D is anticipated to be determined in Q2 2024.
